摘要
目的探讨第四代HIV电化学发光诊断试剂初筛试验假阳性的原因。方法收集HIV抗体初筛试验阳性但确证试验阴性的患者84例,及同期健康体检者88例,对其实验室指标进行比较,探讨引起HIV抗体假阳性的原因。结果HIV抗体假阳性组的WBC、AST、LDH、HBDH水平显著高于健康对照组,差异有统计学意义(P<0.05),HIV抗体假阳性组的TC、TG、HDL-C、LDL-C水平显著低于健康对照组,差异有统计学意义(P<0.05),Spearman相关性分析显示,COI值与WBC(r=0.307)、AST(r=0.336)、LDH(r=0.335)、HBDH(r=0.321)呈正相关,与TC(r=-0.263)、HDL-C(r=-0.281)、LDL-C(r=-0.245)呈负相关,差异有统计学意义(P<0.05)。多元线性回归分析显示AST、HBDH、HDL-C与COI值独立相关,β值分别为0.043、0.027、-2.410。结论WBC、AST、LDH、HBDH升高、血脂相关指标的降低与第四代HIV电化学发光诊断试剂初筛试验假阳性关系密切。
Objective To explore reasons of false-positive results in the primary screening test of the fourth generation HIV electrochemiluminescence diagnostic reagent.Methods 84 cases of HIV antibody primary screening test positive but confirmatory test negative and 88 physical examination samples were selected for the study.We compared these samples’laboratory indicators and explored potential causes of HIV false-positive.Results Levels of WBC,AST,LDH,and HBDH in the HIV antibody false-positive group were significantly higher than those in the healthy control group(P<0.05).Levels of TC,TG,HDL-C,and LDL-C in the HIV antibody false-positive group were significantly lower than those in the healthy control group(P<0.05).Spearman correlation analysis showed that COI values were positively correlated with WBC(r=0.307),AST(r=0.336),LDH(r=0.335),and HBDH(r=0.321),while negatively correlated with TC(r=-0.263),HDL-C(r=-0.281),and LDL-C(r=-0.245),with statistically significant differences(P<0.05).Multiple linear regression analysis showed that AST,HBDH,HDL-C were independently correlated with COI values,with the valuesβof 0.043,0.027,and-2.410,respectively.Conclusion Levels of WBC,AST,LDH,HBDH increase,and the blood lipid related indicators decrease,which are closely related to the false-positive results in the primary screening test of the fourth generation HIV electrochemiluminescence diagnostic reagent.
