摘要
目的 分析子宫肌瘤超声指标与MRI T2WI信号强度的相关性,预测聚焦超声消融治疗的难易程度。资料与方法 回顾性分析2020年5月—2021年7月遂宁市中心医院聚焦超声消融治疗前238例单发子宫肌瘤患者的经阴道超声及MRI T2WI图像,获取子宫肌瘤T2WI信号强度类型与超声指标;采用Logistic回归分析超声指标与T2WI信号强度的相关性,通过受试者工作特征曲线分析超声指标对T2WI高信号肌瘤的预测价值。结果 238例患者T2WI信号表现:高信号组86例、非高信号组152例。子宫肌层与肌瘤灰度值差(Z=6.025,P<0.001)、能量多普勒血流信号像素面积比率(Z=5.957,P<0.001)、黏膜下肌瘤(Z=15.882,P=0.047)是不同T2WI信号强度的相关因素;使用受试者工作特征曲线验证Logistic回归模型,曲线下面积为0.817,约登指数为0.51,敏感度为0.826,特异度为0.684。结论 超声指标可在聚焦超声消融术前预测MRI T2WI高信号子宫肌瘤,具有一定的临床实用价值。
Purpose To analyze the correlation between ultrasound index and MRI T2WI signal intensity of uterine fibroids,and to predict the degree of difficulty in the treatment of focused ultrasound ablation for uterine fibroids.Materials and Methods Two hundred and thirty-eight patients with a solitary uterine fibroid were retrospectively enrolled in Sichuan Suining Central Hospital from May 2020 to July 2021.The ultrasound indexes of fibroids and types of signal intensity in MRI T2WI were obtained from the images of transvaginal sonography and MRI before focused ultrasound ablation treatment.Logistic regression analysis was used to analyze the correlation between ultrasound indexes and signal intensity in T2WI.Receiver operating characteristic curve was used to analyze the predictive value of ultrasound indexes for hyperintense fibroids on T2WI.Results In 238 patients,86 patients had a hyperintense fibroid,and 152 patients had an non-hyperintense fibroid.Logistic regression analysis showed those ultrasound indexes included grayscale value difference between myometrium and fibroids(Z=6.025,P<0.001),ratio of pixel area of power Doppler flow signal(Z=5.957,P<0.001)and submucous leiomyoma(Z=15.882,P=0.047)were related to signal intensity in T2WI.The Logistic regression model was validated by receiver operating characteristic curve.The area under the curve was 0.817.The Youden index was 0.51.The sensitivity was 0.826,and the specificity 0.684.Conclusion A model with ultrasound indexes for signal intensity in T2WI can be used to predict the hyperintense of uterine fibroids on MRI T2WI before focused ultrasound ablation treatment,which has clinical practical value.
