摘要
目的研究取消耗材加成政策和耗材集中带量采购政策对患者住院费用及结构的影响。方法利用2017—2020年四川省两次耗材政策改革前后的心脏介入类手术患者住院医疗费用数据,构建双断点回归模型,分析耗材政策对患者住院费用的影响。结果四川省两次耗材政策改革在一定程度上遏制了医疗费用的增长,住院总费用、康复费、手术用一次性耗材费、耗材费用占比和药占比在两次政策改革后均有不同程度下降(P<0.05),取消耗材加成政策使得费用结构进一步优化。结论取消耗材加成政策和带量采购政策的实施使心脏介入手术患者的住院总费用下降,患者经济负担减轻。手术费用呈现上涨趋势的同时手术耗材费用降低,表明医生的技术劳务价值得到提升,应持续巩固取消耗材加成政策与带量采购政策成果。
Objective To study the effects of the policy of medical consumables zero-markup reform and the medical consumables group procurement policy on patients’inpatient costs and structure.Methods A double-cutoffs regression discontinuity design was constructed to analyze the impact of the consumables policy on patients’inpatient medical costs using data on inpatient medical costs of patients undergoing coronary interventional procedures before and after the two reforms of consumables policy in Sichuan Province from 2017 to 2020.Results The two reforms of consumables policy in Sichuan Province curbed the growth of medical costs to a certain extent,and the total inpatient costs,rehabilitation costs,surgical disposable consumables costs,consumables cost ratio and drug ratio all decreased to different degrees after the two reforms(P<0.05).The medical consumables zero-markup reform led to further optimization of the cost structure.Conclusion The medical consumables zero-markup reform and medical consumables group procurement policy led to a decrease in the total hospital costs of patients undergoing cardiac interventions and a reduction in the financial burden on patients.The trends of increasing surgical costs and decreasing costs of surgical consumables indicate that the value of physicians’technical services has been enhanced.
