摘要
骨质疏松与骨折是老年人常见病,骨折手术需要结合老年人的特点来选择麻醉方式。腰硬联合麻醉具有较强的持续镇痛效果、用药剂量较少、可完全肌松、较完善的神经阻滞效果、起效迅速等诸多优势。本文简述了椎管内麻醉在老年人中应用具有肺部并发症较少、经济便捷等优势,也介绍了蛛网膜下腔麻醉在老年人中应用具有麻醉剂量较大、平面扩散迅速、广泛,对循环的影响较大等诸多不足,同时介绍了腰硬联合麻醉具有操作便捷、镇痛起效快速、迅速达到麻醉效果等诸多优势,为老年下肢骨折患者手术麻醉方式的选择提供参考依据。
Osteoporosis and fracture are common diseases in the elderly,and the anesthesia method should be selected according to the characteristics of the elderly in fracture operation.Lumbo-epidural anesthesia has many advantages,such as strong sustained analgesic effect,less dosage,complete muscle relaxation,perfect nerve blocking effect,rapid onset and so on.This article briefly described the advantages of intraspinal anesthesia in the elderly,such as less pulmonary complications,economic and convenient,and also introduced the shortcomings of subarachnoid anesthesia in the elderly,such as large anesthetic dose,rapid planar diffusion,extensive,and great influence on circulation.At the same time,the paper introduced the advantages of lumbo-epidural anesthesia,such as convenient operation,rapid analgesic effect and rapid anesthesia effect,which provides a reference for the selection of surgical anesthesia methods for elderly patients with lower extremity fracture.
