摘要
目的 探讨血小板、白细胞计数联合C反应蛋白诊断小儿过敏性紫癜肾脏损害的临床意义。方法 选择2020年1月—2022年11月无锡市儿童医院收治的过敏性紫癜患儿80例为研究对象。按照是否合并肾脏功能损伤,分为观察组(合并肾脏功能损伤)和对照组(无肾脏功能损伤),各40例。比较两组血小板、白细胞和C反应蛋白水平,分析其诊断小儿过敏性紫癜肾脏损害的临床意义,绘制其单独及联合诊断小儿过敏性紫癜肾脏损害的受试者工作(receiver operator characteristic curve, ROC)曲线,计算曲线下面积(area under the curve, AUC)。结果 观察组血小板、白细胞和C反应蛋白水平均显著高于对照组,差异有统计学意义(P<0.05),血小板、白细胞和C反应蛋白三者联合诊断小儿过敏性紫癜肾脏损害的灵敏度和特异度则均超过90.0%,约登指数接近0.85,AUC面积为0.952 8。结论 过敏性紫癜肾脏功能损伤患儿,存在显著的血小板、白细胞计数及C反应蛋白水平升高,血小板、白细胞计数及C反应蛋白水平三者联合检测诊断过敏性紫癜肾脏功能损伤具有较理想的诊断价值。
Objective To investigate the clinical significance of platelet and white blood cell count combined with C-reactive protein in the diagnosis of renal damage in children with anaphylactoid purpura.Methods A total of 80 chil‐dren with anaphylactoid purpura treated in Wuxi Children's Hospital from January 2020 to November 2022 were se‐lected as the study objects.The patients were divided into observation group(with kidney function injury)and control group(without kidney function injury),with 40 cases each.The levels of platelets,white blood cells and C-reactive protein were compared between the two groups to analyze their clinical significance in the diagnosis of renal damage in children with anaphylactoid purpura.Receiver operator characteristic(ROC)curve was drawn for diagnosis of renal damage in children with anaphylactoid purpura alone and combined,area under the curve(AUC)area was calculated.Results The levels of platelet,leukocyte and C-reactive protein in observation group were significantly higher than those in control group,and the difference was statistically significant(P<0.05).The sensitivity and specificity of plate‐let,white blood cell and C-reactive protein in the diagnosis of renal damage in children were more than 90.0%,the Jorden index was close to 0.85,and the AUC area was 0.9528.Conclusion In children with anaphylactoid purpura re‐nal function injury,there are significant increases in platelet count,white blood cell count and C-reactive protein level.The combined detection of platelet count,white blood cell count and C-reactive protein level has an ideal diag‐nostic value.
