摘要
目的:探讨老年人自评健康状况与医疗服务可及性之间的关系,为制定医疗服务可及性相关政策、提高老年人健康水平提供参考依据。方法:采用分层随机抽样方法,利用自制问卷在山东省威海市、潍坊市、菏泽市抽取1787名老年人进行问卷调查,利用二分类logistic回归对老年人自评健康状况的影响因素进行分析。结果:老年人自评健康状况为健康的有1440人,不健康的有344人,自评健康状况率为80.72%。二分类logistic回归分析显示,获得交通工具较容易、医疗费用下降、就医时选择社区医院、每年进行1次健康体检的老年人自评健康状况较好(均P<0.05)。结论:交通可获得性、诊疗费用变化、选择医疗卫生机构的类型和健康体检是影响老年人自评健康状况的重要因素,在经济可及性中,除考虑经济支持外,还需考虑精神支持对老年人健康的影响。
Objective:To explore the relationship between the self-assessment of health status of older adults and the accessibility of medical services,so as to formulate policies related to medical service accessibility and improve the health level of older adults.Methods:Using stratified random sampling method,the self-made questionnaire was used to conduct a questionnaire survey among 1787 older adults in Weihai,Weifang and Heze of Shandong province,and the influencing factors of self-rated health of older adults were analyzed by binary logistic regression.Results:1440 older adults were rated as healthy and 344 as unhealthy,with a self-rated health rate of 80.72%.Binary logistic regression analysis showed that older adults who had easier access to transportation,medical cost decreased,chose community hospitals when seeking medical treatment,and underwent annual health examinations had higher self-assessment health status(all P<0.05).Conclusion:Transportation accessibility,change in medical expenses,selection of medical and health institution type and health examination are important factors affecting the self-rated health status of older adults.In economic accessibility,in addition to economic support,the impact of spiritual support on the health of older adults should also be considered.
