摘要
目的探究互助小组式同伴支持饮食管理在慢性心力衰竭(chronic heart failure,CHF)患者中的应用效果。方法选取2021年6月至2022年6月在我院治疗的CHF患者87例,将所有患者根据随机数表分为两组,其中按照常规干预方法进行护理的为对照组,有43例。在对照组基础上行互助小组式同伴支持饮食管理干预的,为观察组,有44例。在干预前后,测定评价营养指标,评价患者的饮食依从性和临床症状转归情况,并评价患者的生活质量和自我管理能力。结果干预后,各指标均有改善,且观察组患者血清白蛋白(Albumin,ALB)、前白蛋白(Prealbumin,PAB),上臂肌围(Arm muscle circumference,AMC)、上臂肱三头肌皮褶厚度(Triceps skinfold,TSF)等营养指标较对照组改善更为明显(P<0.05);在干预后,两组患者的饮食依从性较干预前均有改善,观察组患者评分为(68.93±6.63)分,高于对照组的(58.41±6.01)分;干预后,观察组患者pro-BNP水平为(2563.31±203.62)ng/L,低于对照组干预后的(4426.59±256.47)ng/L;观察组患者LVEF水平为(56.48±6.51)%,高于对照组干预后的(51.16±5.97)%;干预后,观察组患者在自我管理能力评分方面为(70.39±6.63)分,高于对照组的(55.47±5.37)分;观察组生活质量评分为(52.36±5.51)分,高于对照组干预后的(41.28±4.08)分;上述比较差异有统计学意义(P<0.05)。结论对CHF患者实施互助小组式同伴支持饮食管理,患者的营养状况得到改善,饮食依从性增加,预后得以提高,值得临床推广。
Objective To examine the effect of mutual assistance group style peers support dietary management in patients with chronic heart failure.Methods Eighty-seven patients with chronic heart failure(CHF)treated in our hospital from June 2021 to June 2022 were selected,with one group receiving care according to the routine intervention method as control group with 43 patients.Another group had a mutual assistance group style peer support diet management intervention on their basis as the observation group with 44 patients.Before and after the intervention,nutritional indicators were measured to evaluate patients'dietary compliance and clinical symptom outcomes,as well as their quality of life and self-management ability.Results After intervention,all indicators were improved,and the nutritional indicators such as albumin(ALB),prealbumin(PAB),arm muscle circumference(AMC),and triceps skinfold(TSF)in the observation group were more significantly improved than those in the control group(P<0.05).After intervention,the dietary compliance of patients in both groups improved compared to before intervention.The score of patients in the observation group was(68.93±6.63)points,which was higher than that of the control group(58.41±6.01)points.After intervention,the level of pro-brain-natriureticpeptide(pro-BNP)in the observation group was(2563.31±203.62)ng/L,which was lower than that in the control group(4426.59±256.47)ng/L.The LVEF level in the observation group was(56.48±6.51)%,higher than that in the control group(51.16±5.97)%.After intervention,the score of self-management ability in the observation group was(70.39±6.63),higher than that in the control group(55.47±5.37).The score of quality of life in the observation group was(52.36±5.51),which was higher than that in the control group(41.28±4.08).The above comparative differences were statistically significant(P<0.05).Conclusion The implementation of mutual assistance group style peer support diet management for CHF patients resulted in improved nutritional status of patients,increased dietary compliance,and improved prognosis,which merits clinical promotion.
