FLAIR血管高信号征对急性缺血性脑卒中的临床预后及侧支循环的相关性研究 被引量:3

摘要 目的探讨单侧大脑中动脉供血区急性缺血性脑卒中患者MRI-FLAIR序列血管高信号(FVH)与患者侧支循环及临床预后的关系。方法选取因大脑中动脉闭塞导致急性脑梗塞患者82例,FVH征阳性者70例,收集患者基本临床资料,根据MRI-FLAIR及CTP图进行FVH评分和侧支血管分级,根据90天mRS评分分为预后良好组(mRS≤2分)及预后不良组mRS(3~6)分。采用Pearson相关分析评估FVH与90天mRS评分及梗死面积的相关关系,Spearman秩相关检验评估FVH与侧支血管评分的相关关系;采用单因素分析、多变量二元Logistic回归分析及ROC曲线分析,探讨影响AIS患者预后相关因素。结果Spearman秩相关检验结果显示FVH评分与侧支血管评分呈正相关关系,Pearson分析结果显示FVH与90天mRS评分及梗死面积呈负相关关系。多变量二元Logistic回归分析的结果显示,FVH评分、侧支血管评分和梗死体积与AIS患者的预后相关,差异有统计学意义。FVH评分高的患者具有更小的梗死体积和更好地临床预后。结论FVH评分与侧支血管评分呈正相关关系,FVH评分可表示侧支循环情况;FVH评分与90天mRS评分、梗死体积呈负相关。FVH可预示临床预后。 Objective To investigate the relationship between FLAIR sequence high signal intensity(FVH)and collateral circulation and clinical prognosis in patients with acute ischemic stroke.Methods 70 patients with acute cerebral infarction caused by middle cerebral artery occlusion were retrospectively collected.Basic clinical data were collected.FVH and collateral vessel grading were performed according to MRI-FLAIR and CTP images.According to the 90-day Mrs Score,the patients were divided into good prognosis group(mRS≤2)and bad prognosis group(mRS3~6).Pearson Correlation Analysis was used to evaluate the correlation between FVH and 90-day mRS.Spearman rank correlation test was used to evaluate the correlation between FVH and Collateral Vessel Score and infarct size.Single factor analysis,multivariate Logistic regression analysis and ROC curve analysis were used to explore the related factors affecting the prognosis of AIS patients.Results Spearman rank correlation test showed a positive correlation between FVH and collateral vessel score,while Pearson analysis showed anegative correlation between FVH and 90-day mRS Score and infarct size.Multivariate binary Logistic regression analysis showed that FVH and infarct volume were significantly correlated with the prognosis of AIS patients.Patients with high FVH scores had smaller infarct volume and better clinical prognosis.Conclusion FVH score is positively correlated with the score ofcollateral vessels,and FVH score is negatively correlated with 90-day mRS score and infarct volume.FVH Can predict the clinical prognosis.
