摘要
目的:分析彩色多普勒超声在良恶性乳腺结节中的鉴别诊断效能。方法:选取2021年4月—2022年4月高唐县人民医院收治的97例乳腺结节患者,所有患者均行彩色多普勒超声检查,以病理检查结果为“金标准”,统计分析彩色多普勒超声在乳腺结节良恶性中的诊断效果。结果:97例乳腺结节患者中,病理检查42例恶性,55例良性,彩色多普勒超声共检出40例恶性,良性57例。以病理检查为“金标准”,彩色多普勒超声检查的灵敏度为92.86%(39/42)、特异度为98.18%(54/55)、准确度为95.88%(39+54)/97、阳性预测值为97.50%(39/40)、阴性预测值为94.74%(54/57),Kappa=0.916,一致性极好;良性结节的形态不规则、边界不清晰、血流丰富、有钙化占比低于恶性乳腺结节,差异有统计学意义(P<0.05);良性乳腺结节的搏动指数(pulsatility index,PI)、阻力指数(resistance index,RI)、收缩期峰值流速(peak systolic velocity,PSV)低于恶性乳腺结节,差异有统计学意义(P<0.05)。结论:彩色多普勒超声在良恶性乳腺结节鉴别诊断中效果显著,且不同性质的乳腺结节其影像特征与血流参数存在较大差异。
Objective To analyze the differential diagnosis efficiency of color Doppler ultrasound in benign and malignant breast nodules.Methods Ninety-seven patients with breast nodules admitted to Gaotang County People's Hospital from April 2021 to April 2022 were selected.All patients underwent color Doppler ultrasonography.Pathological examination result was taken as the"gold standard"to analyze the diagnostic effect of color Doppler ultrasonography in benign and malignant breast nodules.Results Among 97 patients with breast nodules,42 cases were malignant and 55 cases were benign by pathological examination Color Doppler ultrasound detected 40 malignant cases and 57 benign cases.Taking pathological examination as the"gold standard",the sensitivity of color Doppler ultrasonography was 92.86%(39/42),specificity 98.18%(54/55),accuracy 95.88%(39+54)/97,positive predictive value 97.50%(39/40)and negative predictive value 94.74%(54/57).Kappa=0.916,excellent consistency;The benign nodules had irregular shape,unclear boundary,rich blood flow and a lower proportion of calcification than malignant breast nodules(P<0.05).The PI,RI and PSV of benign breast nodules were lower than those of malignant ones(P<0.05).Conclusions Color Doppler ultrasound has a significant effect in the differential diagnosis of benign and malignant breast nodules,and there are significant differences in imaging characteristics and blood flow parameters among different types of breast nodules.
