摘要
目的:分析子宫颈冷刀锥切术(CKC)与环形电切术(LEEP)治疗子宫颈上皮内瘤变(CIN)的临床效果。方法:选取2018年1月—2023年1月泰安市东平县接山镇中心卫生院收治的52例CIN患者作为研究对象,随机分为LEEP组与CKC组,各26例。LEEP组采取LEEP进行治疗,CKC组采取CKC进行治疗。对比两组手术治疗指标、激素水平、应激反应因子水平、并发症发生情况。结果:LEEP组手术时间、住院时间、子宫颈愈合和月经恢复时间均短于CKC组,术中出血量少于CKC组,差异有统计学意义(P<0.001)。术后,LEEP组黄体生成素、孕激素、睾酮水平高于CKC组,促卵泡激素、雌二醇水平低于CKC组,差异有统计学意义(P<0.05)。术后,LEEP组超氧化物歧化酶、总抗氧能力水平高于CKC组,丙二醛、超敏C反应蛋白水平低于CKC组,差异有统计学意义(P<0.05)。LEEP组并发症总发生率低于CKC组,差异有统计学意义(P=0.018)。结论:LEEP治疗CIN较CKC有明显的优势,创伤较小、手术时间较短、术后恢复快,且利于维持患者的激素水平,减轻应激反应,降低并发症发生风险,应用价值较高。
Objective:To analyze clinical effect of cervical cold knife conization(CKC)and loop electrosurgical excision procedure(LEEP)in treatment of cervical intraepithelial neoplasia(CIN).Methods:A total of 52 patients with CIN who were admitted to Taian City Dongping County Jieshan Town Central Health Center from January 2018 to January 2023 were selected as the study subjects.They were randomly divided into LEEP group and CKC group with 26 cases in each group.LEEP group was treated with LEEP,and CKC group was treated with CKC.The surgical treatment indexes,hormone levels,stress response factor levels,and complications in the two groups were compared.Results:The operation time,hospital stay,cervical healing time and menstrual recovery time in LEEP group were shorter than those in CKC group,intraoperative blood loss in LEEP group was less than that in CKC group,and the difference was statistically significant(P<0.001).After surgery,the levels of luteinizing hormone,progesterone and testosterone in the LEEP group were higher than those in the CKC group,and the levels of follicle-stimulating hormone and estradiol in the LEEP group were lower than those in the CKC group,and the difference was statistically significant(P<0.05).After surgery,the levels of superoxide dismutase and total antioxidant capacity in the LEEP group were higher than those in the CKC group,and the levels of malondialdehyde and hypersensitivity C-reactive protein in the LEEP group were lower than those in the CKC group,and the differences were statistically significant(P<0.05).The total incidence of complications in the LEEP group was lower than that in the CKC group,and the difference was statistically significant(P=0.018).Conclusion:LEEP has obvious advantages over CKC in treating CIN.It has slighter trauma,shorter operation time,faster recovery after operation,and is conducive to maintaining the hormone level,reducing stress response level,lowering the risk of complications,with high application value.
