摘要
目的系统评价近十年发布的子宫内膜异位症指南,通过对其进行质量评价和比较,为子宫内膜异位症临床诊疗策略的选择提供参考。方法使用计算机搜索,从2012年12月至2022年6月的PubMed数据库、Embase数据库、美国循证医学临床实践与指导数据库、英国国家健康与临床医学优选研究所(NICE)检索出公开发表的、由专业机构或组织发布的子宫内膜异位症临床实践指南(CPG)。由两位研究者按照开发指南研究和评估工具Ⅱ(AGREEⅡ)标准分别对被纳入的CPG项目作出质量评价,评价内容主要分为CPG的范围与目的、参加人员、CPG的严谨性、制定的清晰性、应用性、独立性等6个领域,而CPG的推荐等级根据对上述6个领域的标准化评分的结果确定。结果一共纳入了8条子宫内膜异位症的CPG,其中包括指南5条,共识3条;范围涵盖了两大洲5个国家;结果发布的年份为2013年至2022年,评分的结果依次是:77.1%、52.8%、50.5%、86.8%、31.3%、36.5%。8篇CPG中,A级(推荐)1篇,B级(完善后推荐)5篇,C级(不推荐)2篇。结论2013年—2022年发布的子宫内膜异位症CPG质量差别较大,部分CPG在严谨性、应用性、独立性方面不够理想。2017年NICE发布的指南为A级推荐,各领域的标准化评分均较高,其CPG的形成过程也最为规范。
Objective To systematically evaluate the quality of the endometriosis guidelines issued in recent ten years,and to provide reference for the clinical diagnosis and treatment strategy of endometriosis through quality evaluation and comparison.Methods From December 2012 to June 2022,PubMed database,Embase database,EBM clinical practice guidelines database and national institute of health and clinical excellence(NICE)database were searched by computer,and the clinical practice guidelines(CPG)published by professional institutions or organizations on endometriosis were retrieved.The quality of the CPG included was evaluated by two researchers according to the appraisal of guidelines for research&evaluationⅡ(AGREEⅡ).The evaluation content includes six areas:scope and purpose of CPG,participants,rigor of CPG,clarity of expression,applicability,and independence.The CPG recommendation level is determined by the distribution of standardized scores in the above six areas.Results A total of 8 endometriosis CPGS were included,including 5 guidelines and 3 consensuses.It covers 5 countries in 2 continents;the result were published from 2013 to 2022,the average standardized scores in the six areas of CPG were 77.1%,52.8%,50.5%,86.8%,31.3%,and 36.5%,respectively.Among the 8 CPG,1 was graded A(recommended),5 were graded B(recommended after improvement),and 2 were graded C(not recommended).Conclusions The quality of endometriosis CPG published from 2013 to 2022 differs greatly from each other,and some CPG are not ideal in terms of rigor,application and independence.The guidance issued by NICE in 2017 is Grade A recommendation,with high standardized scores in all areas,and the CPG formation process is the most standardized.
