无创呼吸机结合雾化吸入在老年重症肺心病合并呼吸衰竭患者中的应用效果 被引量:1

Application Effect of Non-invasive Ventilator Combined with Atomization Inhalation in Elderly Patients with Severe Pneumocardial Disease Complicated with Respiratory Failure

摘要 目的:探讨无创呼吸机结合雾化吸入在老年重症肺心病合并呼吸衰竭患者中的应用效果。方法:选取2019年2月—2021年12月南华大学附属第二医院收治的80例老年重症肺心病合并呼吸衰竭患者为研究对象。根据随机数表法将其分为联合组和参照组,各40例。参照组给予常规治疗及氧疗,联合组给予常规治疗及无创呼吸机结合雾化吸入。比较两组症状缓解时间,治疗前后心功能指标、肺功能指标、血气指标及并发症。结果:联合组呼吸困难、胸闷心悸、剧烈咳嗽缓解时间均早于参照组,差异有统计学意义(P<0.05)。治疗后,联合组左心室射血分数(LVEF)高于参照组,氨基末端脑钠肽前体(NT-proBNP)水平低于参照组,差异有统计学意义(P<0.05)。治疗后,联合组第1秒用力呼吸容积(FEV_(1))、FEV_(1)/用力肺活量(FEV_(1)/FVC%)和最大呼气流量(PEF)均高于参照组,差异有统计学意义(P<0.05)。治疗后,联合组pH值、氧分压(PaO_(2))、血氧饱和度(SaO_(2))均高于参照组,二氧化碳分压(PaCO_(2))低于参照组,差异有统计学意义(P<0.05)。联合组并发症发生率(2.50%)明显低于参照组(20.00%),差异有统计学意义(P<0.05)。结论:无创呼吸机结合雾化吸入在老年重症肺心病合并呼吸衰竭患者中的应用效果显著,可明显提高患者心功能和肺功能,改善血气指标,减少并发症。 Objective:To explore the application effect of non-invasive ventilator combined with atomization inhalation in elderly patients with severe pneumocardial disease complicated with respiratory failure.Method:A total of 80 elderly patients with severe pneumocardial disease complicated with respiratory failure admitted to the Second Affiliated Hospital of University of South China from February 2019 to December 2021 were selected as the study objects.According to random number table method,they were divided into combination group and reference group,with 40 cases in each group.The reference group was given conventional treatment and oxygen therapy,and the combination group was given conventional treatment and non-invasive ventilator combined with atomization inhalation.The symptom relief time,cardiac function index,lung function index,blood gas index before and after treatment and complications were compared between two groups.Result:The relief time of dyspnea,chest tightness and palpitations,and severe cough in combination group were earlier than those in reference group,and the differences were statistically significant(P<0.05).After treatment,the left ventricular ejection fraction(LVEF)in combination group was higher than that in reference group,and the level of amino-terminal brain natriuretic peptide precursor(NT-proBNP)was lower than that in reference group,the differences were statistically significant(P<0.05).After treatment,the forced expiratory volume in one second(FEV_(1)),FEV_(1)/forced vital capacity(FEV_(1)/FVC%)and maximum expiratory flow(PEF)in the combination group were higher than those in the reference group,the differences were statistically significant(P<0.05).After treatment,the pH value,oxygen partial pressure(PaO_(2))and blood oxygen saturation(SaO_(2))of combination group were higher than those of reference group,and the partial pressure of carbon dioxide(PaCO_(2))was lower than that of reference group,the differences were statistically significant(P<0.05).The incidence of complications in combination group(2.50%)was significantly lower than that in reference group(20.00%),and the difference was statistically significant(P<0.05).Conclusion:The application effect of non-invasive ventilator combined with atomization inhalation in elderly patients with severe pneumocardial disease complicated with respiratory failure is remarkable,which can significantly improve the heart and lung function of patients,improve blood gas index and reduce complications.
