摘要
目的:观察疏肝益肾起痿汤联合他达拉非片治疗5-羟色胺再摄取抑制剂(SSRIs)类抗抑郁药所致勃起功能障碍(ED)患者的效果。方法:选取112例肝郁肾虚型SSRIs类抗抑郁药所致ED患者,采用随机数字表法分为2组各56例,对照组给予他达拉非片治疗,观察组给予疏肝益肾起痿汤联合他达拉非片治疗,2组均治疗8周。对比2组临床疗效,治疗前后评定中医证候评分、勃起功能[勃起硬度评价量表(EHS)评分、国际勃起功能评分量表(IIEF-5)评分]、心理状态[抑郁症筛查量表(PHQ-9)评分]、血管内皮功能[血清内皮素-1(ET-1)、一氧化氮(NO),血管扩张功能(FMD)],观察治疗期间的不良反应。结果:治疗后,观察组临床疗效优于对照组(P<0.05)。2组性欲下降、阴茎痿软不举、阴茎举而不坚、忧愁善感、小便淋漓评分及PHQ-9评分均较治疗前降低(P<0.05),观察组6项评分均低于对照组(P<0.05)。2组EHS评分、IIEF-5评分均较治疗前升高(P<0.05),观察组EHS评分、IIEF-5评分均高于对照组(P<0.05)。2组ET-1水平均较治疗前降低,NO水平及FMD值均较治疗前升高,差异均有统计学意义(P<0.05)。观察组ET-1水平低于对照组,NO水平及FMD值均高于对照组,差异均有统计学意义(P<0.05)。2组患者均未发生明显不良反应。结论:采用疏肝益肾起痿汤联合他达拉非片治疗肝郁肾虚型SSRIs类抗抑郁药所致ED效果明显,能够改善患者的心理状态和血管内皮功能,提高勃起功能,安全性高。
Objective:To observe the effect of Shugan Yishen Qiwei Decoction combined with Tadalafil Tablets on patients with erectile dysfunction(ED)caused by antidepressants of selective serotonin reuptake inhibitors(SSRIs)type.Methods:A total of 112 cases of patients with ED caused by SSRIs antidepressants of liver depression and kidney deficiency type were selected and divided into two groups according to the random number table method,with 56 cases in each group.The control group was treated with Tadalafil Tablets,and the observation group was treated with Shugan Yishen Qiwei Decoction combined with Tadalafil Tablets.Both groups were treated for eight weeks.The clinical effects were compared between the two groups.Before and after treatment,traditional Chinese medicine syndrome scores,erectile function[Erection Hardness Scores(EHS),International Index of Erectile Function-5(IIEF-5)scores],psychological state[Patient Health Questionnaire-9(PHQ-9)scores],vascular endothelial function[endothelin-1(ET-1)and nitric oxide(NO)in serum,flow-mediated dilatation(FMD)]were evaluated.During the treatment,adverse reactions were observed.Results:After treatment,the curative effect in the observation group was better than that in the control group(P<0.05).The scores of libido decrease,penile impotence,penile erection but not hard,grief and sentimentality,and dribbling urination,as well as PHQ-9 scores in the two groups were decreased when compared with those before treatment(P<0.05),and the above six scores in the observation group were lower than those in the control group(P<0.05).The scores of EHS and IIEF-5 in the two groups were increased when compared with those before treatment(P<0.05),and the scores of EHS and IIEF-5 in the observation group were higher than those in the control group(P<0.05).The levels of ET-1 in the two groups were decreased when compared with those before treatment,and the levels of NO and the values of FMD were increased when compared with those before treatment,differences being significant(P<0.05).The level of ET-1 in the observation group was lower than that in the control group,and the level of NO and the value of FMD were higher than those in the control group,differences being significant(P<0.05).No obvious adverse reaction occurred in the two groups.Conclusion:Shugan Yishen Qiwei Decoction combined with Tadalafil Tablets for ED caused by SSRIs antidepressants of liver depression and kidney deficiency type has an obvious effect,which can improve psychological state,vascular endothelial function,and erectile function of patients,with high safety.
