摘要
目的探讨阴囊彩超在阴囊急症诊断和治疗中的应用价值。方法回顾性分析本院在2021年1月至2021年12月期间收治的95例阴囊急诊患者的临床资料,所有患者均采用彩色多普勒超声进行检查,比较不同病变类型患者治疗前后的超声影像特征改变情况。结果经彩超检查共诊断出急性附睾睾丸炎59例,急性睾丸扭转23例,急性睾丸附件扭转13例。在睾丸扭转病例中以病理诊断结果为金标准,经彩超检查显示,急性睾丸扭转与病理诊断结果的符合率为100%(23/23),诊断结果的一致性好;治疗后,急性附睾睾丸炎病例中,彩超显示附睾体积增大、血流信号增多、阴囊壁增厚、合并睾丸鞘膜积液情况均较治疗前明显好转(均P<0.05);治疗后,睾丸附件扭转病例中,彩超显示血流信号增多、合并睾丸鞘膜积液、睾丸实质回声异常情况均较治疗前改善(均P<0.05)。结论阴囊彩超检查对阴囊急症的诊断准确率高,不同类型阴囊急症患者经治疗后的超声影像特征、血流分布均呈现明显好转变化,阴囊彩超检查在阴囊急症的诊断及治疗中具有重要价值。
Objective To explore the application value of scrotal color ultrasound in the diagnosis and treatment of scrotal emergencies.Methods Retrospective analysis was performed on the clinical data of 95 patients with emergency scrotum admitted to our hospital from January 2021 to December 2021.All patients were examined by color Doppler ultrasound to compare the changes in ultrasonic image characteristics of patients with different lesion types before and after treatment.Results A total of 59 cases of epididymal orchitis,23 cases of acute testicular torsion and 13 cases of testicular appendage torsion were diagnosed by ultrasonography.In the cases of testicular torsion,pathological diagnosis results were used as the gold standard.Color ultrasound examination showed that the coincidence rate of acute testicular torsion and pathological diagnosis results was 100%(23/23),and the consistency of diagnosis results was good.After treatment,the four indexes of epididymal volume,blood flow signal,scrotal wall thickness and complicated hydrocele of testis were significantly improved in patients with epididymal orchitis by color ultrasound compared with those before treatment(all P<0.05).After treatment,in the cases of testicular adnexal torsion,blood flow signal,hydrocele and abnormal echo of testicular parenchyma were all improved by color ultrasound compared with those before treatment(all P<0.05).Conclusions The diagnostic accuracy of scrotal color ultrasound for scrotal emergencies is high.After treatment,the ultrasonic image characteristics and blood flow distribution of patients with different types of scrotal emergencies show obvious improvement.The scrotal color ultrasound has important value in the diagnosis and treatment of scrotal emergencies.
