摘要
梅尼埃病是临床常见的眩晕,是一种特发性膜迷路积水的内耳病,表现为反复发作的旋转性眩晕、波动性感音神经性听力损失、耳鸣和(或)耳胀满感。其发病机制较为复杂,尚未明确,目前最主要的机制与内淋巴积水有关,且本病多发生于30~50岁年龄段人群。梅尼埃病归属于中医“眩晕”范畴,病因尚不明确,临床上治疗困难,迁延难愈。山西中医药大学附属医院脑病科张晋岳副教授用中西医诊治梅尼埃病有丰富的经验,尤其对从脾胃论治此病有独到的见解,通过补益脾胃来治疗梅尼埃病,以补脾胃之虚为基础,虚实兼顾,通过辨证论治来改善患者的临床症状,增加远期疗效,提高生活质量,取得了良好的治疗效果。文章结合案例介绍张晋岳副教授从脾胃论治梅尼埃病经验,以期为临床提供参考。
Meniere’s disease is a kind of inner ear diseases with idiopathic membranous labyrinth hydrops characterized by recurrent rotational vertigo,undulatory sensorineural hearing loss,tinnitus,and(or)ear fullness.Its pathogenesis is more complex and has not yet been clarified,and the most important mechanism is related to endolymphatic hydrops.The disease usually occurs in young and middle-aged people aged 30 to 50 years.Meniere’s disease belongs to the category of vertigo in TCM.The cause of the disease is still unclear.It is difficult to treat clinically,and it is difficult to cure during prolonged treatment.Associate Professor ZHANG Jinyue from the Department of Encephalopathy of the Affiliated Hospital of Shanxi University of Traditional Chinese Medicine has rich experience in the diagnosis and treatment of Meniere’s disease in TCM and Western medicine,especially has unique views on the treatment of Meniere’s disease from the aspects of spleen and stomach.The professor treated Meniere’s disease by the method of tonifying spleen and stomach,and achieved good therapeutic effects.In this paper,Associate Professor ZHANG Jinyue’s experience in treating Meniere’s disease from the aspects of spleen and stomach is introduced with the case analysis in order to provide a reference for clinical practice.
