摘要
目的:应用中医传承辅助平台分析武汉市中医医院王建华教授中医治疗肾结石的用药规律。方法:收集2021年1月-2022年1月王建华教授治疗肾结石的病例,经筛选后纳入有效处方,建立中药数据库。利用中医传承辅助平台统计药物频次、性味归经,分析组方规律、核心药物组合并挖掘新方。结果:本研究共统计有效处方74首。其中,高频用药16味,包括利尿通淋药(海金沙、金钱草、冬葵子、茯苓、石韦、滑石)、活血化瘀药(白茅根、延胡索、王不留行、莪术)、理气药(枳壳、柴胡)、消食药(鸡内金)、补虚药(白术、白芍)。高频药物组合25组,前三组药物组合是“鸡内金-金钱草”“金钱草-枳壳”“冬葵子-金钱草”。通过熵聚类方法分析得到10个新处方,其中出现频次较高的药物为莲子心、半夏、熟地黄。在所用142味中药中,四气五味与归经分析显示:药性偏寒居多,其次为平和温;五味以甘为主,再次以苦、辛味多见;归经多归为肝、脾、膀胱和胃经。结论:王建华教授认为肾结石多以肾虚为本,夹杂湿热、气滞、瘀血等病理因素;在治疗肾结石时擅长运用甘味药和寒性药物;遣方以清热利湿、利尿通淋药物为主,辅以理气开郁、扶正补虚、活血化瘀的药物。
Objective:To analyze Professor WANG Jianhua’s medication rules of TCM in the treatment of renal calculus by TCM inheritance auxiliary platform.Methods:Cases of renal calculus treated by Professor WANG Jianhua from January 2021 to January 2022 were collected,and the effective prescriptions were included after screening to establish a TCM medicine database.TCM inheritance auxiliary platform was used to summarize the medicine frequency,medicinal property,medicinal taste,and tropism meridian of the included TCM medicine.The prescription rule and core medicine combination were analyzed;and new prescriptions were found.Results:In this study,a total of 74 effective prescriptions were obtained.Among them,there were 16 high-frequency medicines,including diuretic and treating gonorrhea medicines[such as Haijinsha(Spora Lygodiui),Jinqiancao(Herba Lysimachiae),Dongkuizi(Semen Malvae),Fuling(Poria),Shiwei(Folium Pyrrosiae),Huashi(Talcum)],blood activating and stasis removing medicines[such as Baimaogen(Rhizoma Imperatae),Yanhusuo(Rhizoma Corydalis),Wangbuliuxing(Semen Vaccariae),Ezhu(Rhizoma Curcumae)],Qi(气)regulating medicines[such as Zhike(Fructus Aurantii),Chaihu(Radix Bupleuri)],digestion medicines[such as Ji’neijin(Endothelium Corneum Gigeriae Galli)],and nourishing medicines[such as Baizhu(Rhizoma Atractylodis Macrocephalae),Baishao(Radix Paeoniae Alba)].There were 25 groups of high-frequency medicine combinations.The first three groups of medicine combinations were Ji’neijin-Jinqiancao,Jinqiancao-Zhike,Dongkuizi-Jinqiancao.Ten new prescriptions were obtained by the entropy clustering analysis,among which the medicines with higher frequency were Lianzixin(Plumula Nelumbinis),Banxia(Rhizoma Pinelliae)and Shu Dihuang(Radix Rehmanniae Praeparata).Among the 142 TCM medicines used,the analysis of medicinal property,medicinal taste and tropism meridian showed that most of them has cold property,followed by mild and warm properties,that the medicinal taste was mainly sweet,followed by bitter and pungent tastes,and that the main tropism meridians were liver,spleen,bladder and stomach meridians.Conclusion:Professor WANG Jianhua believes that kidney deficiency is the basis of renal calculus,which is mixed with damp heat,Qi stagnation,blood stasis and other pathological factors.He is good at using medicines with sweet taste and cold property to treat renal calculuss.In the prescription,medicines for clearing heat and removing dampness,and diuretic and treating gonorrhea were mainly used,supplemented by medicines for regulating Qi and relieving depression,strengthening vital energy and tonifying body,and promoting blood circulation and removing blood stasis.
