摘要
报告2例持久性发疹性斑状毛细血管扩张症。患者1:男,38岁。前胸、上臂红斑及毛细血管扩张20年。皮肤科检查:前胸、上臂可见密集分布毛细血管扩张性淡红色斑疹,Darier征阴性。上臂皮损组织病理检查:表皮轻度萎缩,真皮浅层玻璃样变,真皮浅层散在小血管扩张,部分小血管及个别附属器周围见少量淋巴细胞、散在肥大细胞浸润。CD117标记示:真皮层小血管周围见散在肥大细胞浸润。诊断:持久性发疹性斑状毛细血管扩张症。予口服酮替芬1 mg每晚1次。患者2:男,49岁。双侧上肢及胸背部红斑毛细血管扩张2年。皮肤科检查:双侧上肢、胸背部分布边界清楚的淡红色斑疹,Darier征阴性。皮损组织病理检查:表皮网篮状角化,真皮浅层小血管周围见散在肥大细胞浸润,间质胶原纤维增生。甲苯胺蓝染色:镜下血管周围见肥大细胞,胞内见紫红色异染颗粒。诊断:持久性发疹性斑状毛细血管扩张症。予口服酮替芬1 mg每晚1次。2例患者电话随访均诉服用酮替芬1个月后皮疹未见明显好转,未再复诊。
Patient one was a 38-year-old man with a 20-year history of red macules and angiotelectasis on the chest and the upper arms.Dermatological examination showed multiple telangiectatic macules on the chest and the upper arms,with a negative Darier's sign.Histopathology revealed mild epidermal atrophy,hyaline degeneration and scattered vasodilation in the superficial dermis.Perivascular infiltration of lymphocytes and mast cells was evident in the superficial dermis.CD117 staining demonstrated perivascular infiltration of mast cells in the dermis.Diagnosis:telangiectasia macularis eruptiva perstans.Patient two was a 49-year-old man with red macules and angiotelectasis on the trunk and the arms for over 2 years.Dermatological examination revealed demarcated erythematous maculopapular rashes on the upper limbs,the chest and the back,with a negative Darier's sign.Histopathological findings included reticular keratosis,perivascular infiltration of mast cells in the superficial dermis,and proliferation of collagen fibers.Toluidine blue staining showed degranulation of perivascular mast cells.The diagnosis was telangiectasia macularis eruptiva perstans.Both patient were treated with oral ketotifen at a dose of 1 mg/evening.They claimed no significant improvement after one-month treatments.
