摘要
目的探讨血浆代谢物与伴T2DM脑小血管病患者认知障碍的相关性。方法选取2021年7月至2022年6月锦州医科大学附属第一医院神经内科伴T2DM脑小血管病住院患者80例,根据有无认知功能障碍分为LIANGNAG组:非认知障碍组(non-cognitive impairment,NCI)、认知障碍组(cognitive impairment,CI),应用串联质谱法方法检测血浆中35个代谢指标,应用独立样本t检验、秩和检验筛选出组间差异性指标(P<0.05),Logistic回归模型分析外周血各指标与认知障碍的关系;ROC曲线检验模型预测疾病的准确性。结果年龄、高血糖、高血压、谷氨酸(glutamic acid,Glu)、同型半胱氨酸(homocysteine,Hcy)、苏氨酸(threonine,Thr)、N 1、酰基肉碱(C3、C5、C10、C18、N 2)等指标可能参与认知障碍的发病机制并对疾病有较好的预测价值。结论以上指标可能参与认知障碍的发病机制,多种代谢物联合预测可提高疾病诊断价值。
Objective To investigate the correlation between plasma metabolites and cognitive impairment in patients with small cerebral vascular disease associated with T2DM.Methods 80 inpatients with T2DM small cerebral vascular disease in the Department of Neurology of the First Affiliated Hospital of Jinzhou Medical University from July 2021 to June 2022 were selected and divided into LIANGNAG group according to the cognitive dysfunction:In the non-cognitive impairment(NCI)and cognitive impairment(CI)groups,35 metabolic indexes in plasma were detected by tandem mass spectrometry.Independent sample t test and rank sum test were used to screen out the differences among groups(P<0.05),and Logistic regression model was used to analyze the relationship between peripheral blood indexes and cognitive impairment.The ROC curve test model was used to predict the accuracy of disease.Results Age,hyperglycemia,hypertension,glutamic acid(Glu),homocysteine(Hcy),threonine(Thr),N 1,acylcarnitine(C3,C5,C10,C18,N 2)and other indicators may be involved in the pathogenesis of cognitive impairment and have good predictive value for the disease.Conclusion The above indexes may be involved in the pathogenesis of cognitive disorders,and the combined prediction of multiple metabolites can improve the diagnostic value of diseases.
