摘要
目的 观察参榆药浴方对肛瘘术后创面愈合和炎症因子、创面生长因子、创伤应激水平的影响。方法 将106例肛瘘术后患者按照随机数字表法分为2组,治疗组53例术后予参榆药浴方熏洗坐浴,对照组53例术后予高锰酸钾液熏洗坐浴,2组均治疗10天。比较2组疗效;比较2组治疗前后创面情况;比较2组治疗前后创面生长因子水平变化;比较2组治疗前后炎症因子水平变化;比较2组治疗前后氧化应激水平变化。结果 治疗组总有效率94.34%(50/53),对照组总有效率84.91%(45/53),治疗组疗效优于对照组(P<0.05)。2组治疗后创面纵径、创面面积均较本组治疗前减少(P<0.05),且治疗组减少更明显(P<0.05)。2组治疗后酸性成纤维细胞生长因子(aFGF)、碱性成纤维细胞生长因子(bFGF)、创面生长因子(EGF)水平均较本组治疗前升高(P<0.05),且治疗组升高更明显(P<0.05)。2组治疗后白细胞介素12(IL-12)、IL-1β、肿瘤坏死因子α(TNF-α)水平均较本组治疗前降低(P<0.05),且治疗组降低更明显(P<0.05)。2组治疗后超氧化物歧化酶(SOD)均较本组治疗前升高(P<0.05),丙二醛(MDA)均较本组治疗前降低(P<0.05);治疗后治疗组SOD高于对照组(P<0.05),MDA低于对照组(P<0.05)。结论 参榆药浴方熏洗坐浴能减轻肛瘘术后患者炎性反应和创伤应激水平,促进肉芽组织生长,加快创面愈合。
Objective To study the effects of Shenyu Medicinal Bath prescription on wound healing,inflammatory factors,epidermal growth factor(EGF)and traumatic stress following anal fistula(AF).Methods A total of 106 patients with AF were randomized 1∶1 to receive fumigation bath of Shenyu Medicinal(the observation group)or fumigation bath potassium permanganate(the control group).The wound healing,inflammatory factors,FGF,and traumatic stress were included as comparisons.Results After treatment,the total effective rate of observation group was better than control group(94.34%[50/53]vs 84.91%[45/53],P<0.05).The longitudinal diameter and area of wound surface in the both groups were smaller than those before treatment(P<0.05),and which in the observation group were smaller than those in the control group(P<0.05).The levels of acidic fibroblast growth factor(aFGF),basic fibroblast growth factor(bFGF),EGF and superoxide dismutase(SOD)in the both groups were increased,which in observation group were more common(P<0.05).The levels of interleukin-12(IL-12),interleukin-1β(IL-1β),tumor necrosis factor-α(TNF-α)and malondialdehyde(MDA)in the both groups were decreased,and which in observation group were more prevalent(P<0.05).Conclusion For patients with AF,Shenyu medicinal bath prescription can reduce postoperative inflammatory reaction and traumatic stress,promote the growth of granulation tissue,and effectively speed up wound healing.
