摘要
目的:探讨四维超声联合经腹部超声心动图在胎儿先天性心脏病(congenital heart disease,CHD)诊断中的临床价值。方法:选取2021年1月—2022年6月于阳谷县人民医院行CHD筛查的1453例孕妇,均开展经腹部超声心动图检查、四维超声检查,统计CHD检出情况及分布类型,以妊娠终止后检查结果为金标准,分析经腹部超声心动图检查、四维超声联合经腹部超声心动图诊断胎儿CHD的临床价值。结果:1453例孕妇金标准显示22例胎儿CHD,四维超声联合经腹部超声心动图诊断胎儿CHD的灵敏度、准确率及阴性预测值均高于经腹部超声心动图检查,差异有统计学意义(P<0.05);四维超声联合经腹部超声心动图对不同疾病类型检出准确率为95.45%,高于经腹部超声心动图的63.64%,差异有统计学意义(P<0.05)。结论:四维超声联合经腹部超声心动图可提高胎儿CHD诊断灵敏度、准确率,降低漏诊、误诊风险,便于早期针对性干预,实现优生优育。
Objective To explore the clinical value of four-dimensional ultrasound combined with transabdominal echocardiography in the diagnosis of fetal congenital heart disease(CHD).Methods A total of 1453 pregnant women who underwent CHD screening in Yanggu County People's Hospital from January 2021 to June 2022 were selected.Abdominal echocardiography and four-dimensional ultrasound were performed to analyze the detection and distribution of CHD.The results of post-termination examination were used as the gold standard.To analyze the clinical value of transabdominal echocardiography,four-dimensional ultrasound combined with transabdominal echocardiography in diagnosing fetal CHD.Results The gold standard for 1453 pregnant women showed 22 cases of fetal CHD.The sensitivity,accuracy and negative predictive value of four-dimensional ultrasound combined with transabdominal echocardiography were higher than those of transabdominal echocardiography,and the difference was statistically significant(P<0.05).The detection accuracy of four dimensional ultrasound combined with transabdominal echocardiography was 95.45%,which was higher than 63.64%of transabdominal echocardiography,the difference was statistically significant(P<0.05).Conclusion The combination of four-dimensional ultrasound and transabdominal echocardiography can improve the sensitivity and accuracy of fetal CHD diagnosis,reduce the risk of missed diagnosis and misdiagnosis,facilitate early targeted intervention,and achieve eugenics and fertility.
