摘要
例1男74岁,例2男60岁,2例患者均因肺鳞癌应用帕博利珠单抗治疗。用药后2例患者均表现为四肢水肿性靶形红斑、丘疹、斑丘疹,合并口唇黏膜糜烂、结痂。皮损组织病理表现为表皮内散在坏死角质形成细胞,表真皮界面不清,真皮乳头水肿,真皮浅层密集淋巴组织细胞、少量噬黑素细胞浸润,血管扩张充血,红细胞外渗,符合多形红斑型药疹。2例患者停用帕博利珠单抗并经系统糖皮质激素等对症治疗后病情控制。
Case 1 was a 74-year-old male and case 2 was a 60-year-old male,both of them were treated with Pabolizumab for lung squamous cell carcinoma.After medication,two patients showed edema target-shaped erythema,papules and maculopapules of the limbs,combined with erosion and scab of the lip mucosa.The pathological manifestations of the skin lesions were scattered in the epidermis with necrotic keratinocytes,unclear surface dermal interface,dermal papillary edema,dense lymphoid tissue cells in the superficial dermis,a small amount of melanocyte infiltration,vasodilation and congestion,erythrocyte extravasation,consistent with erythema multiforme type drug eruption.The condition of two patients was controlled after discontinuation of parbolizumab and symptomatic treatment such as systemic glucocorticoid.
