子宫内膜息肉组织中基质细胞衍生因子-1、信号转导与转录激活子1表达水平与患者息肉切除术后发生宫腔粘连的关系 被引量:2

Relationship between the expression levels of stromal cell-derived factor-1 and signal transducer and activator of transcription 1 in endometrial polyp tissue and intrauterine adhesions after polypectomy

摘要 目的探讨基质细胞衍生因子-1(SDF-1)、信号转导与转录激活子1(STAT1)在行子宫内膜息肉切除手术患者息肉组织中的变化,及其与息肉切除术后发生宫腔粘连的关系。方法选取2019年1月至2021年6月在琼海市人民医院行子宫内膜息肉切除手术的338例患者作为研究对象,收集术后切除的子宫内膜息肉组织,实时荧光定量聚合酶链反应(qRT-PCR)法对组织中SDF-1、STAT1表达水平进行检测;术后进行随访,根据检查结果分为粘连组(发生宫腔粘连,n=101)和未粘连组(未发生宫腔粘连,n=237)。采用受试者工作特征(ROC)曲线评价子宫内膜息肉组织中SDF-1和STAT1水平对术后发生宫腔粘连的诊断价值;采用多因素Logistic回归分析法分析息肉切除术后发生宫腔粘连的影响因素。结果粘连组患者子宫内膜息肉组织中SDF-1、STAT1 mRNA的表达水平均低于未粘连组(P<0.05);Logistic回归分析结果显示,STAT1、SDF-1、孕次、剖宫产史、刮宫史、盆腔炎史、息肉直径均是影响息肉切除术后宫腔粘连发生的独立危险因素(P<0.05);ROC曲线结果显示,STAT1、SDF-1水平预测子宫内膜息肉患者息肉切除术后发生宫腔粘连的曲线下面积分别为0.909、0.811,对应的灵敏度分别为84.16%、75.25%,特异度分别为87.76%、77.64%,二者联合预测的曲线下面积为0.959,灵敏度为96.04%,特异度为89.45%。结论与未发生宫腔粘连患者相比,发生宫腔粘连患者子宫内膜息肉组织中SDF-1、STAT1表达均降低,且二者是息肉切除术后发生宫腔粘连的独立危险因素,二者联合检测可用于预测术后宫腔粘连的发生。 Objective To inv estigate the level changes of stromal cell.derived factor-1(SDF-1)and signal transducer and activator of transeription 1(STATI)in polyp tissue of patients with endometrial polypectomy,and their correlation with intrauterine adhesions after polypectomy.Methods A total of 338 patients who underwent endometrial polypectomy in Qionghai People's Hospital from January 2019 to June 2021 were selected as the research objects,and postoperatively resected endometrial polyp tissue were ollected,and the expression levels of SDF-1 and STATI in the tissues were detected by real-time fluorescence quantitative reverse transcription polymerase chain reaction(qRTPCR).Patients were fllowed-up postoperatively and separated into adhesion group(with intrauterine adhesions,n=101)and non-adhesion group(no intrauterine adhesions,n=237)according to the examination results.Receiver operating characteristic(ROC)curve was performed to evaluate the diagnostic value of SDF-1 and STATI levels in endometrial polyps for postoperative intrauterine adhesions;multivariate Logistic regression analysis was performed to analyze the influencing factors of intrauterine adhesions after polypectomy.Results The expression levels of SDF-1 and STATI mRNA in the.endometrial polyps in the adhesion group were lower than those in the non-adhesion group(P<0.05);Logistic regression analysis showed that STATI,SDF-1,pregnancy times,history of cesarean section,history of curettage,history of pelvic inflammatory disease,and polyp diameter were independent risk factors for the occurrence of intrauterine adhesions after polypectomy(P<0.05);ROC curve results showed that the areas under the curve of STATI and SDF-1 levels for predicting intrauterine adhesions in patients with endometrial polyps after polypectomy were 0.909 and 0.811,respectively,the corresponding sensitivities were 84.16%and 75.25%,and the specificities were 87.76%,77.64%,respectively,the area under the curve of the combined prediction was 0.959,the sensitivity was 96.04%,and the specificity was 89.45%。Conclusions Compared with patients without intrauterine adhesions,the expression of SDF-1 and STAT1 in endometrial polyp tissue of patients with intrauterine adhesions is decreased,and they are independent risk factors for intrauterine adhesions after polypectomy.Combined detection can be applied to predict the occurrence of postopenative intrauterine adhesions.
