摘要
目的对比隧道式经外周置入中心静脉导管(PICC)置管与常规PICC置管在肿瘤化疗患者中的应用效果。方法选取78例肿瘤化疗患者,按照抽签法将其分为观察组(n=39)和对照组(n=39),观察组采用隧道式PICC置管,对照组采用常规PICC置管。比较2组操作情况(一次性穿刺成功率、一次性置管成功率、操作时间、操作中出血量),带管期间并发症(感染、导管堵塞、导管脱落、机械性静脉炎)发生率,癌症患者生命质量测定量表(FACT-G)评分。结果观察组一次性穿刺成功率显著高于对照组(P<0.05),观察组操作中出血量显著低于对照组(P<0.05),2组一次性置管成功率及操作时间均无显著差异(P>0.05)。2组感染、导管堵塞、导管脱落发生率均无显著差异(P>0.05),观察组机械性静脉炎发生率显著低于对照组(P<0.05)。2组在治疗前PWB、SWB、EWB、FWB评分均无显著差异(P>0.05);治疗后,2组患者PWB、SWB、EWB、FWB评分均大幅度下降,且观察组显著低于对照组(P<0.05)。结论隧道式PICC置管操作情况良好,可有效降低并发症发生率,提高肿瘤化疗患者的生命质量。
Objective To explore the comparative application of tunnel type peripherally inserted central venous catheter(PICC)and conventional PICC catheters in patients with tumor chemotherapy.Methods 78 patients who needed tumor chemotherapy were divided into experimental group(n=39)and control group(n=39)according to random number table.Tunnel PICC catheterization was used in the observation group and routine PICC catheterization was used in the control group,The incidence of complications(ifection,catheter blockage,catheter detachment,mechanical phlebitis)during the period of catheterization was compared with the quality of life of cancer patients in the two groups[FACT-G].Results The success rate of one-time puncture in the observation group was significantly higher than that in the control group(P<0.05),the amount of bleeding in the operation in the observation group was signifcantly lower than that in the control group(P<0.05),the success rate of one-time catheterization and the operation time in the two groups were not significantly dfferent(P>0.05),the incidence of infection,catheter blockage and catheter shedding in the observation group and the control group were not significantly diferent(P>0.05),the incidence of mechanical phlebitis in the observation group was significantly lower than that in the control group(P<0.05),There was no significant dilerence in PWB,SWB,EWB and FW B scores between the observation group and the control group before treatment(P>0.05).Afer treatment,the PWB,SWB,EWB and FWB scores of the two groups decreased signifcantly.and the observation group was signifcantly lower than the control group(P<0.05).Conclusion The operation of tunnel PICC catheterization is good,which can efectively reduce the incidence of complications and improve the quality of life of tumor chemotherapists.
