摘要
目的分析良性乳腺叶状肿瘤(PTB)采用真空辅助旋切(VAE)与开放手术对术后局部复发的影响。方法纳入2019年1月-2022年1月湖北文理学院附属谷城医院收治的60例良性PTB患者,利用随机数字表法分为VAE组和开放组,每组30例。VAE组采用真空辅助旋切治疗,开放组则采用开放手术治疗,比较两组手术指标(切口长度、手术时间、术中出血量)、术后恢复情况[术后住院时间、切口愈合时间、温哥华瘢痕量表(VSS)评分]、患者满意度、手术并发症及术后局部复发率。结果VAE组切口长度短于开放组、手术时间短于开放组、术中出血量少于开放组(P<0.05);VAE组术后住院时间、切口愈合时间短于开放组,VSS评分低于开放组(P<0.05);VAE组手术并发症发生率低于开放组(P<0.05);VAE组患者满意度高于开放组(P<0.05);VAE组与开放组术后局部复发率比较,差异无统计学意义(P>0.05)。结论VAE与开放手术对良性PTB均具有确切疗效,其术后局部复发结果相似,但VAE切口更小、手术时间及术后恢复时间更短,且瘢痕影响小、手术并发症少、患者满意度相对更高。
Objective To analyze the effect of vacuum-assisted excision(VAE)and open surgery on local recurrence of benign phyllodes tumor of the breast(PTB).Methods Sixty patients with benign PTB admitted to Gucheng Hospital Affiliated to Hubei University of Arts and Sciences from January 2019 to January 2022 were included and divided into VAE group and open group by random number table method,with 30 patients in each group.The VAE group was treated with vacuum-assisted excision,while the open group was treated with open surgery.The surgical indexes(incision length,operation time,intraoperative blood loss),postoperative recovery[postoperative hospital stay,incision healing time,Vancouver Scar Scale(VSS)score],patient satisfaction,surgical complications and postoperative local recurrence rate were compared between the two groups.Results The incision length of the VAE group was shorter than that of the open group,the operation time was shorter than that of the open group,and the intraoperative blood loss was less than that of the open group(P<0.05).The postoperative hospitalization time and incision healing time in the VAE group were shorter than those in the open group,and the VSS score was lower than that in the open group(P<0.05).The incidence of surgical complications in the VAE group was lower than that in the open group(P<0.05).The satisfaction of patients in the VAE group was higher than that in the open group(P<0.05).There was no significant difference in local recurrence rate between VAE group and open group(P>0.05).Conclusion VAE and open surgery have definite curative effect on benign PTB,and their local recurrence results are similar,but VAE has smaller incision,shorter operation time and postoperative recovery time,less scar effect,fewer surgical complications,and higher patient satisfaction.
