摘要
目的分析内镜下金属钛夹联合兰索拉唑治疗对老年上消化道出血效果及凝血功能的影响。方法方便选取2021年3月—2022年2月淄博市中西医结合医院收治的老年上消化道出血患者84例,按随机数表法分为对照组和研究组,各42例。对照组予以兰索拉唑治疗,研究组予以内镜下金属钛夹联合兰索拉唑治疗,对比两组凝血功能、血清内毒素与C反应蛋白水平、并发症情况。结果治疗后,研究组凝血酶原时间(11.57±0.91)s、凝血活酶时间(28.97±2.20)s长于对照组,纤维蛋白原(2.33±0.46)g/L、血小板(167.23±12.31)×10^(9)/L低于对照组,差异有统计学意义(t=5.026、6.400、9.539、22.576,P<0.05);研究组治疗后血清内毒素(21.44±3.50)μg/L与C反应蛋白(5.14±0.82)mg/L低于对照组,差异有统计学意义(t=29.210、9.194,P<0.05)。两组并发症发生率比较,差异无统计学意义(P>0.05)。结论上消化道出血老年患者用内镜下金属钛夹与兰索拉唑联合治疗,可改善凝血功能,减少并发症的发生,治疗安全性高。
Objective To analyze the effect of endoscopic titanium clip combined with lansoprazole on upper gastrointestinal bleeding and coagulation function in elderly patients.Methods 84 elderly patients with upper gastrointestinal bleeding admitted to Zibo Integrated Traditional Chinese and Western Medicine Hospital from March 2021 to February 2022 were conveniently selected and randomly divided into a control group and a study group by random number table method,with 42 patients in each group.The control group was treated with Lansoprazole,while the study group was treated with endoscopic titanium clips combined with Lansoprazole.The coagulation function,serum endotoxin and C-reactive protein levels,and complications were compared between the two groups.Results After treatment,the prothrombin time(11.57±0.91)s,thrombin time(28.97±2.20)s in the study group were longer than those in the control group,fibrinogen(2.33±0.46)g/L and platelets(167.23±12.31)×10^(9)/L were lower than the control group,the difference was statistically significant(t=5.026,6.400,9.539,22.576,P<0.05);After treatment,serum endotoxin(21.44±3.50)μg/L and C-reactive protein(5.14±0.82)mg/L in the study group were lower than those in the control group,and the difference was statistically significant(t=29.210,9.194,P<0.05).There was no significant difference in the incidence of complications between the two groups(P>0.05).Conclusion The combined treatment of endoscopic titanium metal clips and lansoprazole in elderly patients with upper gastrointestinal bleeding can improve blood coagulation function and reduce complications,and the treatment is safe.
