摘要
目的研究剖宫产患者术后采用不同浓度地佐辛与曲马多联合镇痛的效果与安全性。方法随机选取2020年8月—2022年8月如东县中医院完成剖宫产分娩的患者80例作为研究对象,根据随机抽签结果划分Ⅰ组(26例)、Ⅱ组(27例)、Ⅲ组(27例)。Ⅰ组患者术后镇痛方案为单独用药曲马多;Ⅱ组患者术后镇痛方案为曲马多联合5 mg地佐辛;Ⅲ组患者术后镇痛方案为曲马多联合10 mg地佐辛。分别对3组患者术后不同时间节点疼痛感及镇静效果进行评估,观察统计3组患者发生不良反应的具体表现。结果Ⅱ组、Ⅲ组患者术后不同时间节点静息状态与活动状态下疼痛评分均低于Ⅰ组患者,差异有统计学意义(P<0.05);Ⅱ组、Ⅲ组患者疼痛评分比较,差异无统计学意义(P>0.05)。Ⅰ组患者T1、T2、T3镇静效果评分明显低于Ⅱ组、Ⅲ组患者差异有统计学意义(P<0.05);Ⅱ组患者镇静评分与Ⅲ组比较,差异无统计学意义(P>0.05)。Ⅰ组患者与Ⅱ组患者术后均未出现不良反应,总发生率均为0.00%,Ⅲ组患者术后出现1例尿潴留情况,总发生率为3.70%。3组患者术后发生不良反应的总发生率比较,差异无统计学意义(P>0.05)。结论剖宫产术后采用曲马多联合地佐辛镇痛具有良好效果,其中地佐辛浓度选择5 mg为最佳,在保证镇痛效果的同时,降低减少药量。
Objective To study the analgesic effect and safety of disocine and tramadol combined with different concentration after caesarean section.Methods A total of 80 patients who completed cesarean section in Rudong County Hospital of Traditional Chinese Medicine from August 2020 to August 2022 were randomly selected as research objects,and divided into groupⅠ(26 cases),groupⅡ(27 cases)and groupⅢ(27 cases)according to random drawing results.Postoperative analgesia was tramadol alone in groupⅠ.postoperative analgesia regimen for groupⅡwas tramadol combined with 5 mg desocine,in groupⅢthe postoperative analgesic regimen was tramadol combined with 10 mg desoxine.The pain and sedation effects of patients in the three groups at different time points after surgery were evaluated,and the adverse reactions in the three groups were observed and analyzed.Results The pain scores of patients in groupⅡand groupⅢat different time nodes were lower than those in groupⅠ,and the difference was statistically significant(P<0.05).There was no significant difference in pain score between groupⅡand groupⅢ(P>0.05).T1,T2 and T3 sedative effect scores of group I were lower than those of groupⅡand groupⅢ,and the difference was statistically significant(P<0.05);there was no statistically significant difference in sedation score between groupⅡand groupⅢ(P>0.05).No adverse reactions were observed in groupⅠand groupⅡ,with a total incidence of 0.00%.Urinary retention was observed in 1 case in groupⅢ,with a total incidence of 3.70%.There was no statistically significant difference in the total incidence of postoperative adverse reactions among the three groups(P>0.05).Conclusion The analgesic effect of Tramadol combined with desoxine after caesarean section is good,and 5 mg concentration of desoxine is the best,which can ensure the analgesic effect and reduce the dosage.
