摘要
目的分析老年甲状腺癌患者行甲状腺癌根除术中联合应用颈丛神经阻滞与全身麻醉的临床效果。方法选取2019年4月—2021年4月包头医学院第一附属医院收治的58例老年甲状腺癌患者为研究对象,随机分为两组,每组29例。研究组行甲状腺癌根除术过程中联合应用颈丛神经阻滞与全身麻醉,常规组应用全身麻醉,对比两组患者手术前后激素水平、血流动力学水平,麻醉苏醒质量。结果研究组术后平均动脉压(MAP)较常规组升高水平低,心率(HR)状态更加平稳,差异有统计学意义(P<0.05);研究组术后恢复自主呼吸时间、拔管时间、睁眼时间分别为(10.6±1.3)、(17.6±1.9)、(15.1±1.6)h,均短于常规组的(12.4±2.7)、(19.6±3.3)、(16.6±3.4)h,差异有统计学意义(t=3.235、2.828、2.150,P<0.05)。结论老年甲状腺癌患者行根除术过程中,联合应用颈丛神经阻滞及全身麻醉,能够降低激素水平,保持血流动力学趋于稳定,同时不会影响患者苏醒质量。
Objective To analyze the clinical effect of cervical plexus block combined with general anesthesia in elderly patients with thyroid cancer eradication.Methods 58 elderly patients with thyroid cancer admitted to the First Affiliated Hospital of Baotou Medical College from April 2019 to April 2021 were randomly divided into two groups,with 29 patients in each group.The study group was treated with cervical plexus block and general anesthesia during thyroid cancer eradication,while the routine group was treated with general anesthesia.The preoperative and postoperative hormone levels,hemodynamic levels and the quality of anesthesia recovery were compared between the two groups.Results Postoperative mean arterial pressure(MAP)in the study group was lower than that in the conventional group,and the heart rate(HR)was more stable,the difference was statistically significant(P<0.05).The recovery time of spontaneous respiration,extubation time and eye opening time in the study group were(10.6±1.3)h,(17.6±1.9)h and(15.1±1.6)h respectively,which were shorter than those in the conventional group(12.4±2.7)h,(19.6±3.3)h and(16.6±3.4)h,the difference was statistically significant(t=3.235,2.828,2.150,P<0.05).Conclusion During the eradication of thyroid cancer in the elderly,the combination of cervical plexus block and general anesthesia can reduce the hormone level,keep the hemodynamics stable,and will not affect the quality of recovery.
