摘要
目的 探讨经直肠前列腺穿刺术后至腹腔镜前列腺癌根治术的间隔时间对手术的影响。方法 回顾分析2019-01-01-2022-03-31安徽医科大学附属六安医院泌尿外科完成腹腔镜前列腺癌根治术的82例患者的临床资料,根据经直肠前列腺穿刺术后间隔时间(IT)分为两组:IT≤2周组间隔时间为(12.36±1.65) d, IT≥6周组间隔时间为(47.91±3.88) d。通过比较围手术期指标、术后并发症及预后情况,评价穿刺活检术后间隔时间对腹腔镜前列腺癌根治术影响。结果 IT≤2周组相比IT≥6周组,平均手术时间、平均出血量、输血率、术后引流管保留时间、术后下床时间和术后住院时间差异均无统计学意义,P>0.05;切缘阳性率、术后6个月控尿率及IIEF-5下降值差异均无统计学意义,P>0.05。IT≥6周组患者术前焦虑指数明显高于IT≤2周组,差异有统计学意义,P=0.014。结论 经直肠前列腺穿刺活检术后至腹腔镜前列腺癌根治术的时间间隔长短不是影响该手术难度及效果的主要因素,早期手术反而对患者心理状态有积极作用。
Objective To investigate the effect of the interval time between transrectal prostate puncture and laparoscopic radical prostatectomy on intraoperative difficulty and postoperative outcome.Methods The clinical data of 82 patients who underwent laparoscopic radical prostatectomy in Department of Urology, Liuan Hospital Affiliated to Anhui Medical University from January 2019 to March 2022 were reviewed and comprehensively analyzed.According to the interval time after transrectal prostatectomy, the patients were divided into two groups: the interval time was(12.36±1.65) days in the IT≤2 weeks group and(47.91±3.88) days in the IT≥6 weeks group.The effect of the interval time after needle biopsy on laparoscopic radical prostatectomy was evaluated by comparing the perioperative indicators, postoperative complications and prognosis.Results There were no significant differences in the mean operation time, mean blood loss, blood transfusion rate, drainage tube retention time, postoperative ambulation time and postoperative hospital stay between the IT≤2 weeks group and the IT≥6 weeks group(P>0.05).There were no significant differences in the positive rate of surgical margin, urinary control rate and IIEF-5 decline at 6 months after operation(P>0.05).However, the preoperative anxiety index of patients in IT≥6 weeks group was significantly higher than that in IT≤2 weeks group, and the difference was statistically significant(P=0.014).Conclusions The interval time between transrectal prostate biopsy and laparoscopic radical prostatectomy is not the main factor affecting the difficulty and effect of the operation.On the contrary, early operation has a positive effect on the psychological state of patients.
