摘要
目的探讨熊去氧胆酸胶囊联合健肝乐颗粒治疗自身免疫性肝炎(AIH)对外周血Beclin1及Th1/Th2平衡指标的影响。方法采取随机数字表法将湖北省中医院肝病科2019年2月至2021年3月收治的98例AIH患者分为对照组与观察组,每组49例。两组患者均给予常规泼尼松与硫唑嘌呤免疫抑制剂治疗,对照组在其基础上加用熊去氧胆酸胶囊治疗,观察组在对照组基础上加用健肝乐颗粒治疗,两组均连续治疗4周,比较两组患者临床疗效、肝功能[丙氨酸转氨酶(ALT)、天冬氨酸转氨酶(AST)、总胆红素(TBiL)]、免疫球蛋白指标[免疫球蛋白A(IgA)、免疫球蛋白G(IgG)、免疫球蛋白M(IgM)]、外周血Beclin1水平及Th1/Th2平衡指标[干扰素-γ(INF-γ)、白细胞介素-4(IL-4)、Th1/Th2],同时观察治疗期间不良反应。结果观察组治疗有效率高于对照组,差异有统计学意义(P<0.05)。治疗后观察组与对照组相比AST、ALT及TBiL较低,IgG及IgM较低,外周血Beclin1及INF-γ、Th1/Th2较低,IL-4较高,差异有统计学意义(P<0.05)。两组治疗期间均未出现明显的不良反应。结论熊去氧胆酸胶囊联合健肝乐颗粒治疗AIH可获得较为突出的治疗效果,有效改善肝功能,修复免疫功能,且安全性较高,患者预后表现更好。
Objective To explore the effect of ursodeoxycholic acid capsules combined with Jianganle granules in the treatment of autoimmune hepatitis on peripheral blood Beclin1 and Th1/Th2 balance indexes.Methods A random number table method was used to divide 98 AIH patients admitted to the Department of Hepatology,Hubei Provincial Hospital of Traditional Chinese Medicine from February 2019 to March 2021 into a control group and an observation group,with 49 patients in each group.Both groups of patients were treated with conventional prednisone and azathioprine immunosuppressive agents.The control group was treated with ursodeoxycholic acid capsules on the basis of prednisone.The observation group was treated with Jianganle granules on the basis of the control group.Both groups were continuously treated for 4 weeks.The clinical efficacy,liver function[alanine aminotransferase(ALT),aspartate aminotransferase(AST),total bilirubin(TBiL)],Immunoglobulin indicators[immunoglobulin A(IgA),immunoglobulin G(IgG),immunoglobulin M(IgM)],peripheral blood Beclin1 level,and Th1/Th2 balance indicators[interferon-γ(INF-γ)、Interleukin-4(IL-4),Th1/Th2],and adverse reactions during treatment were compared.Results The clinical effective rate of the observation group was higher than that of the control group,and the difference was statistically significant(P<0.05).Compared with the control group after treatment,the observation group had lower AST,ALT and TBiL,lower IgG and IgM,lower peripheral blood Beclin1 and INF-γ,and higher IL-4,and the difference was statistically significant(P<0.05).No significant adverse reactions were observed during the treatment of both groups.Conclusion Jianursodeoxycholic acid capsules combined with Jianganle granules in the treatment of autoimmune hepatitis can achieve a more prominent therapeutic effect,promote the regulation of the body's liver function,and restore the immune function,and there is no increase in adverse reactions during the regulation of the body's liver function,and the safety is relatively high,the patient's prognosis is better.
