摘要
目的探究血清人附睾蛋白4(HE4)与超声诊断对子宫腺肌病(AD)的预测价值。方法选取2019年6月至2022年6月医院收治的82例疑似AD患者,以病理诊断结果为金标准,评估HE4、超声、HE4联合超声诊断对AD的预测价值。结果82例疑似AD患者中,经病理学确诊46例(试验组),非AD 36例(对照组)。试验组的HE4水平高于对照组,差异有统计学意义(P<0.05);试验组收缩期峰值血流速度(PSV)高于对照组,血流阻力指数(RI)低于对照组,差异有统计学意义(P<0.05)。以病理诊断为金标准,HE4联合超声诊断AD的Kappa=0.91,灵敏度为91.30%,特异度为91.67%,高于HE4单独诊断(Kappa=0.72,灵敏度为69.56%,特异度为75.00%),也高于超声单独诊断(Kappa=0.76,灵敏度为73.91%,特异度为77.78%),差异有统计学意义(P<0.05)。结论对于AD患者,血清HE4与超声诊断均具有一定的诊断效能,但二者联合诊断效能更佳,临床可将两种方式联合用于诊断AD。
Objective The predictive value of serum human epididymal protein 4(HE4)and ultrasound diagnosis for adenomyosis(AD)has been explored.Methods Select 82 suspected AD patients admitted to the hospital from June 2019 to June 2022,and use pathological diagnosis results as the gold standard to evaluate the diagnostic value of HE4,ultrasound,and HE4 combined ultrasound diagnosis for AD.Results Among 82 suspected AD patients,46 were diagnosed with AD through pathological diagnosis(experimental group)and 36 were non AD patients(control group);The HE4 level in the experimental group was higher than that in the control group(P<0.05);The peak systolic velocity(PSV)of the experimental group was higher than that of the control group,and the blood flow resistance index(RI)was lower than that of the control group(P<0.05);Using pathological diagnosis as the gold standard,the Kappa value of HE4 combined with ultrasound in diagnosing AD is 0.91,with a sensitivity of 91.30% and a specificity of 91.67%,which is higher than that of HE4 alone(Kappa=0.72,sensitivity of 69.56% and specificity of 75.00%),and also higher than that of ultrasound alone(Kappa=0.76,sensitivity of 73.91% and specificity of 77.78%).The sensitivity and specificity of combined diagnosis are both higher than that of single diagnosis(P<0.05).Conclusion For AD patients,both serum HE4 and ultrasound diagnosis have certain diagnostic efficacy,but the combined diagnostic efficacy of the two is better.The combination of the two methods can be used in clinical diagnosis of AD.
