摘要
随着社会的发展、人口老龄化的日益加剧,慢性心力衰竭发病率、再住院率居高不下,并出现逐年增高的趋势,严重威胁人类生命健康。慢性心力衰竭是在器质性心系疾病长期存在并持续发展的基础上,在感染、心律失常、前后负荷改变等因素的影响下,致使心肌重塑、心脏的舒张及收缩功能均受到限制,甚至静息状态下的心输出量满足不了机体的需求,从而导致机体缺血、缺氧,形成的一组临床综合征。甘肃天水市中医医院曹宝国主任基于冠状动脉粥样硬化性心脏病从脾论治思想以及多年临床经验认为,慢性心力衰竭病位责之心,涉及五脏,病机以心气、心阳亏虚为本,瘀血、痰浊、水饮为标。针对其本虚标实的病机特点,曹主任提出将“温阳利水,化痰逐瘀”贯彻于治疗始终,同时提倡“五脏兼顾,调畅情志”的治疗思路,并根据患者气血阴阳的盛衰、痰火瘀湿等邪实的偏重立法遣方,促使机体气足血通,水行通畅,瘀消痰尽,从而改善患者症状,延缓疾病的发展,使得心力衰竭得以纠正。
With the development of society and the increasing aging of the population,the incidence of chronic heart failure and the rate of rehospitalization remain high,and there is a trend of increasing year by year,which seriously threatens human life and health.Chronic heart failure is based on the long-term existence and continuous development of organic heart diseases,under the influence of infection,arrhythmia,before and after load changes and other factors,resulting in myocardial remodeling,diastolic and systolic function of the heart are restricted,and even the cardiac output in the resting state can not meet the needs of the body,resulting in ischemia,hypoxia,forming a group of clinical syndromes.Based on the idea of treating coronary heart disease from the spleen and many years of clinical experience,Chief Physician CAO Baoguo believes that the disease of chronic heart failure is in the heart,involving the fiveorgan,the pathogenesis is heart Qi(气),heart Yang(阳)deficiency as the origin,and blood stasis,phlegm turbidity,and retained fluid are the superficial symptoms.According to the characteristics of the pathogenesis of deficient root and excessive superficial,it is proposed to carry out the whole treatment of warming Yang and promoting retained fluid circulation,eliminating phlegm and removing blood stasis.Director CAO Baoguo also put forward the treatment idea of taking into account the five-organ and regulating emotions,and determine the treatment method and prescription according to the rise and fall of Qi,blood,Yin(阴)and Yang of patients,as well as the excessive pathogens such as phlegm,fire,blood stasis and dampness,in order to promote the body's Qi and blood circulation,smooth body fluid metabolism,and eliminate phlegm and blood stasis,so as to ameliorate the symptoms of patients,delay the progression of the disease,and cure heart failure finally.
