摘要
目的分析氯氮平治疗难治性精神分裂症引发中枢神经系统表现异常的影响因素。方法选取2016年1月至2021年12月丽水市第二人民医院收治且使用氯氮平治疗的难治性精神分裂症患者117例,分为中枢神经系统表现异常组72例和正常组45例,比较两组患者临床资料的差异,采用多因素二元logistic回归分析引发中枢神经系统表现异常的影响因素。结果异常组与正常组在是否吸烟、氯氮平治疗前精神分裂症病程、血浆氯氮平浓度以及是否服用碳酸锂方面比较,差异均有统计学意义(均P<0.05)。二元logistic回归分析发现,同时服用碳酸锂治疗是氯氮平治疗难治性精神分裂症引发中枢神经系统表现异常的独立危险因素(P<0.05),氯氮平治疗前精神分裂症病程≥10年是中枢神经系统表现异常的保护因素(P<0.05)。结论精神分裂症病程短、同时服用碳酸锂的难治性精神分裂症患者采用氯氮平治疗后中枢神经系统表现异常的发生风险较大,值得临床医生重视。
Objective To analyze the Influencing factors of central nervous system abnormalities caused by clozapine in the treatment of refractory schizophrenia.Methods A total of 117 patients with refractory schizophrenia who were treated with clozapine in Lishui Second People's Hospital from January 2016 to December 2021 were enrolled in the study,there were 72 cases with abnormal electroencephalogram(abnormal group)and 45 cases with normal electroencephalogram(normal group).The difference of clinical data between the two groups was compared,and the risk factors of abnormal electroencephalogram were analyzed by multivariate binary logistic regression.Results There were significant differences in smoking,duration of schizophrenia before clozapine treatment,plasma clozapine level and co-administration of lithium carbonate between abnormal and normal groups(all P<0.05).The results of binary logistic regression analysis showed that the co-administration of lithium carbonate was an independent risk factor for electroencephalogram abnormality in refractory schizophrenia treated with clozapine(P<0.05),and the duration of schizophrenia over 10 years before clozapine treatment was a protective factor for electroencephalogram abnormality(P<0.05).Conclusion The short course of schizophrenia and the co-administration with lithium carbonate can increase the risk of electroencephalogram abnormalities in refractory schizophrenia patients treated with clozapine.
