摘要
目的:观察基于容积-黏度吞咽测试(V-VST)评估的个性化喂养护理在卒中后吞咽障碍患者中的应用效果。方法:选取2020年7月至2021年2月该院收治的101例卒中后吞咽障碍患者进行前瞻性研究,按照随机数字表法将其分为对照组50例和观察组51例,对照组实施常规管饲护理,观察组在对照组基础上实施基于V-VST评估的个性化喂养护理。比较两组护理前后吞咽功能、营养指标[前白蛋白(PA)、转铁蛋白(TRF)、三头肌皮褶厚度(TSF)、上臂肌围(AMC)]水平、生命质量[吞咽障碍特异性生命质量量表(SWALQOL)]评分、管饲时间和不良事件发生率。结果:护理后,观察组洼田饮水试验分级优于对照组,差异有统计学意义(P<0.05);观察组PA、TRF、TSF、AMC水平和SWAL-QOL评分均高于对照组,管饲时间短于对照组,差异有统计学意义(P<0.05);两组不良事件发生率比较,差异无统计学意义(P>0.05)。结论:在常规管饲护理基础上实施基于V-VST评估的个性化喂养护理可改善卒中后吞咽障碍患者吞咽功能和营养状况,缩短管饲时间,提高生命质量,效果优于单纯常规管饲护理。
Objective:To observe application effects of individualized feeding nursing based on volume-viscosity swallowing test(V-VST)in patients with dysphagia after stroke.Methods:A prospective study was conducted on 101 patients with dysphagia after stroke admitted to this hospital from July 2020 to February 2021.They were divided into control group(50 cases)and observation group(51 cases)according to the random number table method.The control group was given routine tube feeding nursing,while the observation group was given the personalized feeding nursing based on V-VST on the basis of that of the control group.The swallowing function,the nutritional index levels[prealbumin(PA),transferrin(TRF),triceps skinfold thickness(TSF),upper arm muscle circumference(AMC)],the quality of life[dysphagia specific quality of life scale(SWALQOL)]score,the tube feeding time,and the incidence of adverse events were compared between the two groups before and after the nursing.Results:After the nursing,the grade of Kubota drinking water test in the observation group was better than that in the control group,and the difference was statistically significant(P<0.05).The levels of PA,TRF,TSF,AMC and SWAL-QOL score in the observation group were higher than those in the control group,the tube feeding time was shorter than that in the control group,and the differences were statistically significant(P<0.05).However,there was no significant difference in the incidence of adverse events between the two groups(P>0.05).Conclusions:On the basis of the routine tube feeding nursing,the personalized feeding nursing based on V-VST evaluation can improve the swallowing function and the nutritional status of the patients with dysphagia after stroke,shorten the tube feeding time and improve the quality of life.Moreover,it is superior to single routine tube feeding nursing.
