摘要
目的探讨活化激酶C受体1(RACK1)、四个半LIM结构域蛋白1(FHL1)在食管鳞状细胞癌(ESCC)中的表达及与患者预后的关系。方法选取122例局部晚期ESCC患者,取其ESCC组织及癌旁组织,免疫组化法检测RACK1、FHL1的表达情况,并比较不同临床疗效ESCC患者ESCC组织中RACK1、FHL1表达情况。局部晚期ESCC患者预后的影响因素采用多元Logistic回归分析。结果ESCC组织中RACK1、FHL1蛋白阳性表达率均明显低于癌旁组织,差异均有统计学意义(P﹤0.01)。依据实体瘤疗效评价标准,有效87例,无效35例,有效ESCC患者ESCC组织中RACK1、FHL1阳性表达率均高于无效患者,差异均有统计学意义(P﹤0.05)。多因素Logistic分析结果显示,淋巴结转移、分化程度为低分化、肿瘤直径≥5 cm、RACK1阴性表达、FHL1阴性表达均是局部晚期ESCC患者预后死亡的独立危险因素(P﹤0.01)。结论RACK1、FHL1在局部晚期ESCC组织中阳性表达率较低,可作为评估紫杉醇联合顺铂化疗疗效、患者预后的可靠指标。
Objective To explore the expression of receptor for activated C kinase 1(RACK1)and four and a half LIM domains 1(FHL1)in esophageal squamous cell carcinoma(ESCC)and their relationship with patient prognosis.Method A total of 122 locally advanced ESCC patients were selected,and their ESCC tissues and adjacent cancer tissues and adjacent cancer tissues were taken.The expression of RACK1 and FHL1 were detected by immunohistochemistry,and the expression of RACK1 and FHL1 in ESCC tissues of ESCC patients with different clinical efficacy were compared.The prognostic factors of locally advanced ESCC patients were analyzed by multiple Logistic regression analysis.Result The positive expression rates of RACK1 and FHL1 proteins in ESCC tissues were significantly lower than those in adjacent cancer tissues,and the differences were statistically significant(P<0.01).A total of 87 cases were effective and 35 cases were ineffective according to the response evaluation criteria in solid tumor.The positive expression rates of RACK1 and FHL1 in ESCC tissues of effective ESCC patients were higher than those in the ineffective patients,and the differences were statistically significant(P<0.05).Multivariate Logistic analysis showed that lymph node metastasis,poor differentiation,tumor diameter≥5 cm,negative expression of RACK1,and negative expression of FHL1 were independent risk factors for death in patients with advanced ESCC(P<0.01).Conclusion The positive expression of RACK1 and FHL1 in advanced ESCC tissues are low,and they could be reliable indicators for evaluating the efficacy of paclitaxel combined with cisplatin chemotherapy and the prognosis of patients.
