摘要
目的探讨在尿液潜血检验中显微镜红细胞计数、尿液分析仪的应用价值。方法选取2021年6月—2021年12月于本院行尿液潜血检验的110例患者作为研究对象,所有患者尿液样本均行尿液分析仪检验、显微镜红细胞计数检验。对比两种检验方法结果。结果参考《尿液检测的临床解读》为金标准,确诊阳性36例,阴性74例。尿液分析仪结果阳性例数为30例,阳性检出率为27.27%。以金标准作为参照,尿液分析仪潜血检验的假阳性率为23.33%、假阴性率为19.40%,灵敏度为63.89%、特异度为90.54%、阳性预测值为76.67%、阴性预测值83.75%,与金标准的Kappa值为0.652;镜检的假阳性率为8.00%,假阴性率为15.29%,灵敏度为63.89%、特异度为97.30%、阳性预测值为92.00%、阴性预测值84.71%,与金标准的Kappa值为0.812。结论镜检与尿液分析仪各有优势与不足,总体而言,镜检将诊断准确性更高。
Objective To explore the application value of microscope red blood cell count and urine analyzer in urine occult blood test.Methods A total of 110 patients who underwent urine occult blood test in our hospital from June 2021 to December 2021 were selected as the research objects.Urine samples of all patients were tested by urinalysis analyzer and microscope red blood cell count test.The results of the two test methods were compared.Results According to“Clinical Interpretation of urine detection”as the gold standard,36 cases were positive and 74 cases were negative.The number of positive cases by urine analyzer was 30,and the positive detection rate was 27.27%.Taking the gold standard as a reference,the false positive rate and false negative rate of occult blood test of urine analyzer were 23.33%and 19.40%,the sensitivity was 63.89%,the specificity was 90.54%,the positive predictive value was 76.67%and the negative predictive value was 83.75%,and the Kappa value was 0.652.The false positive rate of microscopic examination was 8.00%,the false negative rate was 15.29%,the sensitivity was 63.89%,the specificity was 97.30%,the positive predictive value was 92.00%,the negative predictive value was 84.71%,and the Kappa value compared with the gold standard was 0.812.Conclusion The microscopic examination and urine analyzer have their own advantages and disadvantages.In general,the diagnostic accuracy of microscopic examination is higher.
