摘要
目的比较环泊酚和丙泊酚在无痛胃镜诊疗过程中的临床效果。方法选取2021年9月至2022年5月拟于我院行无痛胃镜的220例患者,按照随机数字表法分为环泊酚组与丙泊酚组各110例,分别采用环泊酚0.4 mg/kg、丙泊酚2 mg/kg进行麻醉诱导,待所有患者BIS数值为40~60且改良警觉/镇静评分≤1时,开始胃镜检查,比较两组注射痛评分、诱导时间、插镜时间、手术时间、药物剂量等情况,以及两组患者苏醒时间、离院时间、麻醉医师和患者满意度评分、不良反应等情况。结果与丙泊酚比较,环泊酚组患者注射痛及不良事件中呼吸暂停、低氧血症发生率明显降低,但体动发生率较高,离院时间稍长(P<0.05);给药3 min后环泊酚组患者平均动脉血压较高(P<0.05);两组患者诱导时间、插镜时间、清醒时间、定向力恢复时间以及医师和患者满意度评分比较,差异无统计学意义(P>0.05)。结论环泊酚可以明显减少注射痛、呼吸暂停、低氧血症等不良事件的发生,并且心血管稳定性更高,在无痛胃镜诊疗过程中的安全性、有效性以及患者和医师的接受度不劣于丙泊酚。
Objective To compare the clinical effects between propofol and cyclopofol in painless gastroscopy.Methods From September 2021 to May 2022,220 patients who were scheduled to undergo painless gastroscopy in our hospital were selected.According to the random number table method,the patients were divided into a cyclopofol group and a propofol group,110 in each group.Anesthesia was induced with cyclopofol 0.4 mg/kg and propofol 2 mg/kg,respectively.Gastroscopy was started when the BIS value of all patients was 40-60 and the Modified Observer′s Assessment of Alert≤1.The injection pain score,induction time,insertion time,operation time and drug dosage were compared between the two groups.At the same time,the time of awakening,time of leaving hospital,score of satisfaction of anesthesiologists and patients,and adverse reactions were also recorded.Results Compared to the propofol group,the injection pain and incidence of apnea and hypoxemia in advance events in the cyclopofol group were significantly reduced,but the incidence of body movement was higher and the time of leaving hospital was longer(P<0.05).The mean arterial pressure(MAP)in the cyclopofol group was higher at the 3rd minute after administration(P<0.05).There was no significant statistical difference in terms of induction time,insertion time,awake time,recovery time of orientation,and physician and patient satisfaction between the two groups.Conclusions Cyclopofol can significantly reduce the injection pain,apnea,hypoxemia,and have higher cardiovascular stability.However,its safety,effectiveness and acceptance of patients and physicians in the painless gastroscopy are not inferior to propofol.
