呼吸道合胞病毒肺炎患儿血清IgE水平及呼出气一氧化氮浓度分析 被引量:2

Analysis of serum IgE level and fractional exhaled nitric oxide concentrations in children with respiratory syncytial virus pneumonia

摘要 目的观察呼吸道合胞病毒肺炎患儿血清免疫球蛋白E(IgE)水平及呼出气一氧化氮浓度变化情况。方法200例呼吸道合胞病毒肺炎患儿,111例患儿用于IgE水平观察,89例患儿用于呼出气一氧化氮浓度观察。IgE入选患儿根据诊断标准分为喘息组(54例)与非喘息组(57例);另选取44例体检健康儿童作为健康对照组。呼出气一氧化氮入选患儿根据诊断标准分为喘息组(49例)与非喘息组(40例);另选取42例体检健康儿童作为健康对照组。比较各组血清IgE水平、呼出气一氧化氮浓度。结果喘息组的血清IgE浓度(24.56±11.02)mg/L高于非喘息组的(11.85±7.70)mg/L及健康对照组的(4.50±3.66)mg/L,非喘息组的血清IgE浓度高于健康对照组,差异有统计学意义(P<0.05)。喘息组呼出气一氧化氮浓度(23.02±7.49)ppb高于非喘息组的(13.90±3.71)ppb及健康对照组的(10.71±3.87)ppb,非喘息组呼出气一氧化氮浓度高于健康对照组,差异有统计学意义(P<0.05)。结论IgE在喘息发生过程中起了一定作用,呼吸道合胞病毒感染后可能引起嗜酸性粒细胞性气道炎症。可以通过检测血清IgE水平及呼出气一氧化氮浓度的变化为感染后早期干预提供指导意见,进而避免发生气道变态反应炎症,通过减轻临床症状促进支气管哮喘病情恢复。 Objective To observe the changes of serum immunoglobulin E(IgE)level and fractional exhaled nitric oxide concentrations in children with respiratory syncytial virus pneumonia.Methods Of the 200 children with respiratory syncytial virus pneumonia,111 children were used for IgE level observation and 89 children were used for fractional exhaled nitric oxide concentration observation.The selected cases of IgE were divided into wheezing group(54 cases)and non-wheezing group(57 cases)according to the diagnostic criteria;44 healthy children who underwent physical examination were selected as healthy control group.The selected cases of fractional exhaled nitric oxide were divided into the wheezing group(49 cases)and the non-wheezing group(40 cases)according to the diagnostic criteria;42 healthy children who underwent physical examination were selected as the healthy control group.The serum IgE levels and fractional exhaled nitric oxide concentrations of each group were compared.Results The serum IgE concentration of(24.56±11.02)mg/L in wheezing group was higher than that of(11.85±7.70)mg/L in non-wheezing group and(4.50±3.66)mg/L in healthy control group;the serum IgE concentration in non-wheezing group was higher than that in healthy control group;the differences were statistically significant(P<0.05).The fractional exhaled nitric oxide concentrations of(23.02±7.49)ppb in wheezing group was higher than that of(13.90±3.71)ppb in non-wheezing group and(10.71±3.87)ppb in healthy control group;the fractional exhaled nitric oxide concentrations in non-wheezing group was higher than that in healthy control group;the differences were statistically significant(P<0.05).Conclusion IgE plays a role in the development of wheezing,and respiratory syncytial virus infection may cause eosinophilic airway inflammation.Changes in the levels of serum IgE and fractional exhaled nitric oxide concentration can be detected to provide guidance for early intervention after infection,which in turn can prevent the occurrence of airway metabolic inflammation and promote good progression of bronchial asthma by reducing clinical symptoms.
