摘要
目的分析多学科协作诊疗式安宁疗护干预对终末期肿瘤患者营养状况、生活质量及家属心理应激反应的影响。方法将126例终末期肿瘤患者根据干预模式的不同分为对照组(传统安宁疗护干预)62例和观察组(多学科协作诊疗式安宁疗护干预)64例。比较干预前后两组患者简易营养评估量表(MNA)、数字分级评分法(NRS)、亲属应激量表(RSS)及健康调查简表(SF-36)评分。结果干预后,两组患者MNA评分及SF-36量表各维度评分均升高,NRS评分均降低,且观察组患者MNA评分及SF-36量表各维度评分均高于对照组,NRS评分低于对照组,差异均有统计学意义(P﹤0.05)。干预后,两组患者家属RSS评分均明显降低,且观察组患者家属RSS评分明显低于对照组,差异均有统计学意义(P﹤0.01)。观察组患者不良反应总发生率低于对照组,差异有统计学意义(P﹤0.05)。结论多学科协作诊疗式安宁疗护干预可有效改善终末期肿瘤患者的营养状况、生活质量及家属心理应激水平。
Objective To evaluate the effect of palliative care intervention based on multidisciplinary collaborative diagnosis and treatment on the nutritional status,quality of life in patients with end-stage cancer and psychological stress response of family members.Method A total of 126 patients with end-stage cancer were divided into control group(n=62,received traditional palliative care intervention)and observation group(n=64,received palliative care intervention based on multidisciplinary collaborative diagnosis and treatment)according to different intervention modes.The scores of mininutritional assessment(MNA),numerical rating scale(NRS),relative stress scale(RSS)and the MOS 36-item short form health survey(SF-36)between two groups before and after intervention were compared.Result After intervention,scores of MNA and dimensional scores of SF-36 in both groups increased,and scores of NRS decreased,with higher MNA and SF-36 scores,and lower NRS score in the observation group than those in the control group(P<0.05);after intervention,the RSS scores of family members of patients in both groups were significantly decreased,with lower RSS score in the observation group than that in the control group(P<0.01);the overall incidence of adverse reactions in the observation group was lower than that in the control group(P<0.05).Conclusion Palliative care intervention based on multidisciplinary collaborative diagnosis and treatment can effectively improve the nutritional status and quality of life in patients with end-stage cancer,and the psychological stress level of family members.
