摘要
目的:分析超声诊断卵巢囊肿蒂扭转的影像学表现,探讨囊壁厚、囊肿位置异常、“漩涡征”、囊肿和卵巢血流消失在超声诊断卵巢囊肿蒂扭转中的可行性及临床意义。方法:纳入2019年9月1日~2022年4月30日经手术证实为卵巢囊肿蒂扭转并于术前有超声诊断的患者共42例作为观察组,并随机抽取上述时间内超声诊断卵巢囊性或囊实性肿块且于本院行手术治疗的女性患者共164例作为对照组。对两组患者的超声特点分析及总结。结果:观察组中,单发囊肿为38例(90.5%)、囊壁厚为18例(42.9%),囊肿位置异常为25例(59.5%)、“漩涡征”为29例(69.0%)、囊肿和卵巢血流消失为15例(35.7%);而对照组中,单发囊肿为114例(69.5%)、囊壁厚为6例(3.7%)、囊肿位置异常为10例(6.1%)、“漩涡征”为6例(3.7%)、囊肿和卵巢血流消失为1例(0.6%),差异均有统计学意义(P<0.01)。结论:囊壁厚、囊肿位置异常、“漩涡征”、囊肿和卵巢血流消失超声表现在诊断卵巢囊肿蒂扭转时具有重要价值,超声诊断显示单发囊肿发生率高于多发囊肿蒂扭转。
Objective:To analyze Ultrasonic characteristics of ovarian cyst torsion,and to explore the feasibility and clinical significance of thick wall,abnormal cyst location,whirlpool sign and disappearance of cyst and ovarian blood flow in the diagnosis of ovarian cyst torsion.Methods:From September 1st,2019 to April 30th,2022,a total of 42 patients with ovarian cyst pedicle torsion confirmed by surgery and diagnosed by ultrasound before operation were selected as the research objects,a total of 164 female patients with ovarian cystic or cystic solid masses diagnosed by ultrasound and operated in our hospital were randomly selected as the control group.Analyze and summarize the ultrasonic characteristics of the two groups of patients.Results:Among 42 ovarian cyst torsion,there were 38cases of single cyst(90.5%),18 cases of thick wall(42.9%),25 cases of abnormal cyst location(59.5%),29 cases of whirlpool sign(69.0%),cyst and egg blood flow disappeared in 15 cases(35.7%);In the control group,there were 114 cases of single cyst(69.5%),6 cases of thick wall(3.7%),10 cases of abnormal cyst location(6.1%),6 cases of whirlpool sign(3.7%),and 1 case of disappearance of cyst and ovarian blood flow(0.6%),the difference was statistically significant(P<0.01).Conclusion:It is valuable to diagnosis of ovarian cyst torsion by the thick wall,the abnormal position,the whirlpool sign,and the disappearance of the blood flow of the cyst and ovary.The incidence of torsion of a single cyst is higher than multiple cysts.
