摘要
目的探讨经尿道等离子双极电切术(PKRP)与经尿道前列腺电切术(TURP)对前列腺增生(BPH)患者尿动力学和勃起功能的影响。方法选取2020年1月至2021年12月诸暨市中医医院收治的192例BPH患者,采用随机数字表法分为PKRP组和TURP组,各96例。记录两组手术情况、术后并发症发生情况,观察两组术前及术后1周尿动力学指标,术后6个月随访两组国际勃起功能指数-5(IIEF-5)评分、国际前列腺症状评分(IPSS)。结果PKRP组手术时间、膀胱冲洗时间及尿管留置时间均短于TURP组,术中出血量少于TURP组(P<0.05);术后1周PKRP组最大尿流率(Q_(max))大于TURP组,残余尿量(RUV)少于TURP组(P<0.05);术后6个月PKRP组IIEF-5评分高于TURP组,IPSS低于TURP组(P<0.05);PKRP组并发症发生率低于TURP组(P<0.05)。结论PKRP可有效恢复BPH患者尿动力学水平,不影响患者勃起功能,并发症少,疗效显著。
Objective To explore the effects of plasmakinetic resection of prostate(PKRP)and transurethral resection of prostate(TURP)on urodynamics and erectile function in patients with benign prostatic hyperplasia(BPH).Methods A total of 192 BPH patients admitted to Zhuji Hospital of Traditional Chinese Medicine from January 2020 to December 2021 were selected and divided into PKRP group and TURP group by random number table method,with 96 cases in each group.The operation status and postoperative complications of the two groups were recorded,and the urodynamic indexes of the two groups were observed before and 1 week after the operation.All were followed up for 6 months after surgery to observe scores of international index of erectile function-5(IIEF-5)and international prostate symptom score(IPSS).Results The operation time,bladder irrigation time and catheter indwelling time in PKRP group were shorter than those in TURP group,and intraoperative blood loss was less than that in TURP group(P<0.05).At 1 week after surgery,the maximum flow rate(Q_(max))in PKRP group was higher than that in TURP group,and residual urine volume(RUV)was less than that in TURP group(P<0.05).At 6 months after surgery,IIEF-5 score in PKRP group was higher than that in TURP group,and IPSS was lower than that in TURP group(P<0.05).The incidence of complications in PKRP group was lower than that in TURP group(P<0.05).Conclusions PKRP can effectively recover urodynamics,and will not affect erectile function of patients,with significant curative effect and few complications.
