摘要
目的探寻糖尿病足溃疡患者中医证型与临床指标的关系,为足溃疡病人的辨证分型提供一个新思路。方法选择140例DFU患者按照指南进行辨证分型,采集实验室指标,同时配以30例单纯糖尿病患者做对照,分析各证型与实验室指标的关系。结果中医证型以气血两虚、脉络瘀阻证(28.6%)最为常见。湿热毒蕴、筋腐肉烂证及热毒伤阴、瘀阻脉络证CRP明显高于其他证型;脾肾阳虚、痰瘀阻络证TC高于其他证型,而ALB低于其他证型。多因素Logistic回归模型显示CRP、ALB是湿热毒蕴、筋腐肉烂证的危险因素;ALB、TC是脾肾阳虚、痰瘀阻络证的危险因素。结论DFU患者中医证型与实验室指标有一定相关性,对DFU的辨证分型具有参考意义。
Objective To explore the relationship between TCM syndrome types and clinical indicators in patients with diabetic foot ulcer(DFU)and provide a new idea for TCM syndrome differentiation of DFU.Methods A total of 140 cases of DFU patients and 30 cases of simple diabetes patients were selected as the control group.DFU patients were classified according to the guidelines,and the clinical laboratory indicators of patients were collected to explore the relationship between TCM syndromes and clinical indicators.Results The most common TCM syndromes of 140 DFU patients were deficiency of Qi and blood and blood stasis in collateral(28.6%).The C-reaction protein(CRP)level of damp-heat-toxin accumulation and muscle-tendon rotting syndrome and heat damging Yin and blood stasis obstructing collateral syndrome was significantly higher than that of the other syndrome types.Total cholesterol(TC)of spleen-kidney Yang deficiency and phlegm-blood stasis obstructing colateral syndrome was higher than that of the other syndrome types,while albumin(ALB)was lower than that of the other syndrome types.Multivariate Logistic regression model showed that CRP,ALB and infection grade were the risk factors of damp-heat-toxin accumulation and muscle-tendon rotting syndrome.TC and ALB are th risk factors for spleen-kidney Yang deficiency and phlegm-blood stasis obstrcting collateral syndrome.Conclusion Common clinical indicators are of reference significance for syndrome differentiation and treatment of DFU.
