摘要
目的系统评价中药治疗儿童睑板腺囊肿的临床疗效和安全性。方法检索中药治疗12岁及以下儿童睑板腺囊肿的随机对照试验文献,检索数据库包括中国生物医学文献数据库(CBM)、中国知网(CNKI)、万方、维普(VIP)、PubMed、Web of Science、Embase、Cochrane Library数据库,以及中国临床试验注册中心、美国临床注册中心,检索时限均自建库到2022年10月。由2名研究者独立进行文献筛选、资料提取、质量评价,用Cochrane风险偏倚评估及改良后Jadad量表对所纳入文献进行风险偏倚评估,并采用Review Manager 5.3软件、Stata 14.1软件进行质量评价和Meta分析,运用GRADE profiler3.6软件对结果进行证据分级推荐标准(GRADE)评价。结果共纳入8篇文献,547例患儿,其中中药治疗组276例,常规药物组271例。经过固定效应量分析结果为:与常规药物治疗比较,中药治疗能提高临床总有效率[OR=3.460,95%CI(2.230,5.390),Z=5.510,P=0.000]、降低复发率[OR=0.160,95%CI(0.070,0.350),Z=4.570,P=0.000],且无严重不良反应。总有效率、复发率的GRADE证据等级均为低级。结论中药治疗儿童睑板腺囊肿比常规药物治疗效果更好,复发率更低。但现阶段文献的样本量相对偏少,文献质量和证据质量等级较低,未来仍需获取大样本、高质量的研究来进一步验证。
OBJCTIVE To systematically evaluate the efficacy and safety of Traditional Chinese Medicine(TCM)in the treatment of children with meibomian cyst.METHODS Through searching China Biology Medicine disc(CBM),China National Knowledge Infrastructure(CNKI),WanFang,VIP,PubMed,Web of Science,Embase,Cochrane Library database,China Clinical Trial Registry and Clinical Trial.gov from their establishment to October 2022,we collected randomized controlled trials(RCTs)of TCM treatment on children aged 12 years old and under with meibomian cyst.Two researchers independently screened articles,extracted data and assessed the quality of included articles by Cochrane bias risk assessment tool and the modified Jadad quality score.Review Manager 5.3 and STATA14.1 were used to conduct quality evaluation and Meta-analysis.And GRADE profiler 3.6 software was employed to assess evidence quality rating of outcomes.RESULTS A total of eight RCTs including 547 children with meibomian cyst were included,276 in TCM group and 271 in the control group.The result of showed that the total effective rate of TCM group was higher than that of the control group[OR=3.460,95%CI(2.230,5.390),Z=5.510,P=0.000].The recurrent rate of TCM group was lower than that of the control group[OR=0.160,95%CI(0.070,0.350),Z=4.570,P=0.000].And it had no severe adverse reactions.Evidence level of the total effective rate and recurrent rate were all low based on GRADE evaluation.CONCLUSIONS TCM treatment is superior to common treatment and has lower recurrent rate in the treatment of children with meibomian cyst.Due to the limited samples and low quality of included articles,it is necessary to further verify those results by enrolling clinical trials with more samples and higher quality in the future.
