摘要
目的比较左侧乳腺癌保乳术后容积弧形调强放疗(VMAT)与调强适形放疗(IMRT)的剂量学差异。方法分别为2019年1月至2020年1月在中山市中医院治疗的15例左侧乳腺癌保乳术后患者进行VMAT和IMRT计划设计。分析评价两种放疗计划靶区的剂量参数(包括适形指数、剂量均匀度、V110%)及危及器官的剂量参数差异。结果VMAT计划的V110%较IMRT计划升高,适形指数较IMRT计划升高,但靶区的剂量均匀性较IMRT计划差(P<0.05)。与IMRT计划相比,VMAT计划增加了患侧肺、双肺及心脏的V5、V10、Dmean,但降低了患侧肺、双肺及心脏的V40,VMAT计划增加了健侧乳腺的Dmean(P<0.05)。结论对于左侧乳腺癌保乳术后辅助放疗,VMAT与IMRT技术各有优劣,在行放疗计划设计前应多因素分析,个体化选择合适的放疗技术。
Objective To compare the dosimetry differences between volumetric modulated arc therapy(VMAT)and intensity modulated radiotherapy(IMRT)for left-sided breast cancer patients undergoing breast-conserving operation.Methods VMAT and IMRT program designs were conducted for 15 left-sided breast cancer patients undergoing breast-conserving operation in Zhongshan Hospital of Traditional Chinese Medicine from January 2019 to January 2020,respectively.The differences in dose parameters of planning target volume(including conformality index,dose uniformity,V110%)and organ at risk dose parameters of the two radiotherapy plans were analyzed and evaluated.Results Compared with the IMRT plan,VMAT plan had higher dosimetry parameters in V110 and conformality index,but worse dose uniformity in the target area(P<0.05).Compared with the IMRT plan,the VMAT plan had higher dosimetry parameters in V5,V10 and Dmean of affected lung,both lungs and heart,lower V40 of them,and the VMAT plan increased Dmean in the healthy breast(P<0.05).Conclusion For left-sided breast cancer patients undergoing breast-conserving operation,both VMAT and IMRT have their own advantages and disadvantages,and a multifactorial analysis should be performed before designing the radiotherapy plan to individually select the appropriate radiotherapy technique.
