摘要
口腔扁平苔藓(oral lichen planus,OLP)是一种常见的口腔黏膜慢性炎症性疾病,可发生癌变,其病因机制尚未研究清楚,目前在临床上尚无法根治,因此OLP的治疗面临巨大挑战。精神应激因素可以引起炎症因子异常变化,导致机体免疫功能失调,在口腔扁平苔藓的发病过程中起到关键的作用。精神心理因素尤其是抑郁、焦虑等精神障碍在口腔扁平苔藓发病和治疗中起到重要调节作用,并且对精神因素相关的口腔扁平苔藓患者进行传统的治疗效果不佳。本文针对与口腔扁平苔藓发病相关的精神因素作一综述,分析精神障碍与口腔扁平苔藓的相关性,以更好了解口腔扁平苔藓发生的病理机制和影响因素,为提高临床治疗效果提供依据。
Oral lichen planus(OLP)is a common chronic inflammatory disease occurring in oral mucosa,which increases the risk of carcinogenesis.The etiological mechanism underlying OLP remains unclear,so it cannot be cured clinically at present,and the treatment of OLP is facing great challenges.Mental stress factors can cause abnormal changes of inflammatory factors and lead to immune dysfunction,which is one of the main causes of OLP.Psychosocial factors,especially depression,anxiety and other mental disorders play an important role in the pathogenesis and treatment of OLP.Moreover,the effect of traditional therapy on patients with OLP associated with mental factors is poor.This article reviews the mental factors related to OLP and analyzes the correlation between mental disorders and oral lichen planus.In order to have a better understanding of the pathological mechanism and affecting factors of OLP,and to lay the fundation for improving the clinical therapy efficacy.
