摘要
目的分析老年血液净化治疗患者非计划性撤机风险因素,并讨论分级护理干预措施。方法选取于我院2020年4月至2022年9月治疗的71例老年血液净化治疗患者,收集其临床资料,包括性别、年龄、体质量管理、血液净化治疗相关参数、护士工作年限等,以Logistic多元回归分析患者非计划性撤机危险因素,讨论并分析分级护理干预措施。结果71例患者中,8例患者发生非计划性撤机;经单因素分析显示,年龄、合并基础病、一次置管成功、发生低体温、发生低血压、发生躁动、护士工作年限均为老年血液净化治疗患者非计划性撤机影响因素(P<0.05);经Logistic多元回归分析显示,年龄、合并基础病、发生低体温、发生低血压、发生躁动为老年血液净化治疗患者非计划性撤机危险因素,一次置管成功、护士工作年限为老年血液净化治疗患者非计划性撤机保护因素(P<0.05)。结论患者基础资料、护士工作质量等均会对老年血液净化治疗患者非计划性撤机产生影响,可针对这一特征进行患者非计划性撤机危险层级划分,并实施针对性防控。
Objective To analyze the risk factors of unplanned withdrawal in elderly patients with blood purification therapy,and discuss the intervention measures of graded nursing.Methods A total of 71 elderly patients with blood purification treatment in our hospital from April 2020 to September 2022 were selected,and their clinical data were collected,including gender,age,weight management,parameters related to blood purification treatment and nurses'working years.The risk factors of unplanned withdrawal of patients were analyzed by Logistic multiple regression,and graded nursing intervention measures were discussed and analyzed.Results Among 71 patients,8 patients had unplanned withdrawal.Univariate analysis showed that age,complicated underlying diseases,one-time catheterization success,hypothermia,hypotension,agitation and working years of nurses were all influencing factors for unplanned withdrawal of elderly patients undergoing blood purification therapy(P<0.05).Logistic multiple regression analysis showed that age,combined with underlying diseases,hypothermia,hypotension and agitation were risk factors for unplanned withdrawal of elderly patients undergoing blood purification treatment,and success of one-time catheterization and working years of nurses were protective factors for unplanned withdrawal of elderly patients undergoing blood purification treatment(P<0.05).Conclusion Patients'basic data and nurses'work quality can affect the unplanned withdrawal of elderly patients after blood purification treatment.According to this feature,the risk levels of unplanned withdrawal of patients can be classified and targeted prevention and control can be implemented.
