摘要
目的探讨中老年人群甘油三酯葡萄糖乘积(TyG)指数与DM发病风险的相关性。方法选取2011年5月至2012年9月于兰州市3个社区40~75岁居民建立队列人群,于2014年6月至2015年11月进行平均3.35年随访,共纳入随访资料完整的非DM人群3768名。比较不同BMI中老人群,各TyG指数四分位数组,第1组(Q1,TyG指数5.11~8.36,n=942)、第2组(Q2,TyG指数8.37~8.69,n=942)、第3组(Q3,TyG指数8.70~9.03,n=942)、第4组(Q4,TyG指数9.04~10.86,n=942)DM发病情况,采用COX比例风险模型回归分析不同BMI中老年人群TyG指数与DM发病的相关性。结果随访约3.35年,新发DM患者274例(7.3%)。总体及正常体重、超重和肥胖人群中,随着TyG指数升高,DM发生率呈递增趋势。COX回归分析显示,校正多种因素后,Q3、Q4组DM发病风险较Q1组增加(P<0.05)。按照BMI分层后,在肥胖人群中Q3、Q4组DM发病风险显著增加,在正常体重、超重和低体重人群中不显著。结论高水平TyG指数是中老年人群DM发病的危险因素,肥胖中老年人群DM发病风险进一步升高。
Objective To investigate the relationship between triglyceride-glucose(TyG)index and incidence risk of diabetes mellitus(DM)in middle-aged and elderly people.Methods A total of 3768middle-aged and elderly individuals without DM were selected as the cohort study population from 3 communities in Lanzhou From May 2011 to September 2012.All the participants were followed up for an average of 3.35years from June 2014 to November 2015.The incidences of DM were compared among different TyG index quartile(Q1:TyG≤8.36,n=942,Q2:8.37≤TyG≤8.69,n=942,Q3:8.70≤TyG≤9.03,n=942,Q4:TyG≥9.04,n=942)groups with different levels of BMI.The relationship between TyG index and incidence risk of DM in middle-aged and elderly people with different levels of BMI was analyzed using Cox regression analysis.ResultsDuring the follow-up of 3.35 years,274(7.3%)subjects developed diabetes.The incidence of DM increased with the increase of TyG index in whole population,as well as in participants with normal body weight,overweight and obesity.Cox regression analysis showed that compared with the TyG index Q1 group,the incidence risk of diabetes significantly increased in the TyG index Q3 and Q4 groups after adjusting relevant factors.When stratified by BMI,the TyG index Q3 and Q4 groups had an significantly increased risk of DM in obesity individuals,while not in subjects with normal weight,overweight or underweight.ConclusionHigh TyG index level was an independent risk factor for DM in middle-aged and elderly residents,especially in subjects with obesity.
