摘要
患者女,28岁,腹痛10 d,全身散在红斑、风团伴痒伴发热4 d。体检:体温38℃,精神欠佳,急性面容,上腹部有广泛压痛,无反跳痛。皮肤科情况:面颈部、躯干及四肢可见广泛分布大小不一、形状不规则的片状红斑、风团。实验室检查:血常规中白细胞计数及中性粒细胞计数升高,淋巴细胞计数下降;血淀粉酶及尿淀粉酶升高;CRP升高;大便涂片呈现二度菌群失调改变。诊断:急性荨麻疹并发高淀粉酶血症。予以抗感染、抗组胺、抑胰酶及护胃等治疗后患者痊愈。
A 28-year-old female had abdominal pain for 10 days, scattered erythema, wheals with itching and fever for 4 days. Physical examination found body temperature 38 ℃,poor spirit, acute facial features, extensive tenderness and no rebound pain in the upper abdomen. Dermatological examination showed irregular erythema and wheals scattered in the face, neck, trunk and limbs. Laboratory examination revealed increased white blood cell count, neutrophil count and decreased lymphocyte count in blood routine;elevation of blood amylase and urine amylase;elevated CRP.The second degree of maladjustment of microflora was observed in stool smear. Acute urticaria with hyperamylasemia was diagnosed. The patient was cured after anti-infection, anti-histamine, suppression of trypsin, stomach protection and other treatments.
