摘要
目的探讨铂类药物所致药品不良反应(ADR)的特点和规律,为临床制定安全用药方案提供帮助。方法收集盐城市第一人民医院2012年1月至2022年7月上报的246份关于铂类药物ADR报告,采用回顾性分析方法,对患者的基本信息、使用药品的名称、原患疾病、ADR发生时间、累及器官或系统、临床表现、ADR严重程度及转归情况进行统计分析,并选择关联规则方法挖掘ADR发生的关联信息。结果246份报告中51~60岁中患者占比最多;最常见的ADR事件为血液系统损害;ADR发生时间大于2 d与血液系统损害关联性较强;ADR发生时间为1~24 h与胃肠系统损害关联性较强;ADR发生时间为1~24 h,患者发生ADR严重程度较低;奥沙利铂导致的ADR严重程度多为一般,安全性较高。结论医护人员和药师在日常工作中应重视ADR的监测和上报,探讨ADR发生的规律,针对性地提出用药监护方案,避免ADR的发生,保障患者用药安全。
Objective To investigate the characteristics and regularity of adverse drug reactions(ADR)in the use of platinum drugs,and to provide reference for clinical formulation of safe drug regimens.Methods A total of 246 ADR reports on platinum drugs reported by Yancheng First People′s Hospital from January 2012 to July 2022 were collected.Retrospective analysis was used to analyze the basic information of patients,drug name,underlying disease,ADR occurrence time,involved organs or systems,clinical manifestations,ADR severity and prognosis,and the association rules were used to mine the association information of ADR occurrence.Results Among the 246 reports,patients aged 51 to 60 accounted for the largest proportion;the most common ADR event was blood system damage;ADR occurrence time more than 2 days was strongly correlated with blood system damage;ADR occurrence time of 1~24 h was strongly associated with gastrointestinal damage;when the ADR occurrence time was within 1~24 h,the severity of ADR in patients was low;the severity of ADR caused by oxaliplatin was mostly normal,and the safety was high.Conclusion Medical staff and pharmacists should pay attention to the monitoring and reporting of ADR in their daily work,discuss the regularity of ADR occurrence,and put forward a targeted medication monitoring plan to avoid the occurrence of ADR and ensure the safety of patients′medication.
