摘要
目的改善指動脈逆行島狀皮瓣的靜脈回流和重建皮瓣的感覺功能。方法改良皮瓣設計:(1)在皮瓣上加帶一皮膚蒂;(2)把皮瓣上的神經纖維束與指神經終末枝斷端吻合。結果20例順利成活,沒有出現靜脈回流障礙的明顯徵象,吻合神經的皮瓣的二點分辨覺在4mm以内。結論改良的指動脈逆行島狀皮瓣比常規的同類皮瓣更安全可靠,皮瓣的感覺功能更好。
Objective To improve the venous drainage of the reverse digital artery flap and innervate the flap.Methods Improvement in the flap design:(1)To add a 3〜4mm wide skin pedicle to the flap;(2)To perform the microneurorrhaphy between the attached sensory nerve of the flap and the digital nerve.Results The flaps of 20 cases have no obvious signs of obstruction of venous regurgitation.The two-point discrimination of the innervated flap is within 4 mm.Conclusions The improved reverse digital artery flap is safer and more reliable than the standard one.The sensory function of the flap is better than noninnervated flaps.
