摘要
目的:探讨腰硬联合麻醉在剖宫产术中的临床效果。方法:选取2021年1-12月冕宁县第二人民医院收治的行剖宫产术的患者156例为观察对象,按照随机数字表法分成对照组和观察组,各78例。对照组给予硬膜外麻醉,观察组给予腰硬联合麻醉,比较两组产妇的麻醉指标时间、血流动力学指标[心率(HR)、舒张压(DBP)、收缩压(SBP)、平均动脉压(MAP)]、不良反应发生率。结果:观察组麻醉诱导时间、最高麻醉阻滞平面时间、手术时间短于对照组,差异有统计学意义(P<0.001);术后24 h,两组产妇HR、DBP、SBP、MAP水平均降低,观察组低于对照组,差异有统计学意义(P<0.001);观察组不良反应发生率低于对照组,差异有统计学意义(P<0.05)。结论:剖宫产术中行腰硬联合麻醉,可缩短麻醉诱导时间、最高麻醉阻滞平面时间、手术时间,缓解血流动力学指标,抑制麻醉应激反应,不良反应较少,效果优良。
Objective:To explore the clinical effect of combined spinal and epidural anesthesia in cesarean section.Methods:One hundred and fifty-six patients admitted to Mianning County Second People's Hospital for cesarean section from January to December 2021 were selected as observation subjects and divided into control group and observation group according to the random number table method,78 cases each.The control group was given epidural anesthesia and the observation group was given combined spinal and epidural anesthesia.The anesthesia index time,hemodynamic indexes[heart rate(HR),diastolic blood pressure(DBP),systolic blood pressure(SBP),mean arterial pressure(MAP)],and incidence of adverse reactions were compared between the two groups.Results:The anesthesia induction time,highest anesthesia block plane time,and operation time were shorter in the observation group than in the control group,and the differences were statistically significant(P<0.001).At 24 h after surgery,the maternal HR,DBP,SBP,and MAP levels were reduced in both groups,and the observation group was lower than the control group.The difference was statistically significant(P<0.001).The incidence of adverse reactions in the observation group was lower than that in the control group,and the difference was statistically significant(P<0.05).Conclusion:Combined spinal and epidural anesthesia during cesarean section can shorten the induction time of anesthesia,the time of the highest anesthetic blocking plane,and the operation time,alleviate the hemodynamic index,and suppress the anesthetic stress reaction,with fewer adverse reactions and excellent results.
